Case of a woman whose shoe size increased from 5 to 7 over two years

1. It was an usual OPD, when I was counselling one of my patients with back pain about exercise, diet and correct posture. Incidentally, I noticed his wife's face who was sitting beside the patient.

2. Her nose & lips appeared a bit larger than usual. When she smiled, her teeth were little spaced out and tongue was also little larger than normal. She spoke with a little heavy voice.
Being a #neurologist, I was imagining the possibility of a brain #disease.

3. I asked her- "Don't mistake me, but has your shoe size increased?" Her eyes almost popped out. She replied with a shocked expression- "doc, yes! It has increased from 5 to 7 in the past two years. Don't everyone's feet become larger as they grow older?"
She was 37-year-old now

Observation - no swollen lymph nodes, no temp, still mild cough (I had it since September), is my permanent tinnitus from jabs gone or disappearing?, still strange feeling with my heart, not really tired, couldn’t sleep, will probably stay in bed #COVID19

2) Comparison:
Size of nodes: 1st jab - hazelnut, 2nd jab - walnut, booster - golf ball
Covid - my nodes are normal size.

Tinnitus: 1st - stayed for 2-3 weeks, 2nd - a month, booster - permanent
Covid - hours without it

3) Vision after vaccines
My vision in the right eye deteriorated: blurry, can't focus, grey/black spots when coughing, takes more effort to move my eye, squinting a lot.
My long distance was always perfect on the right eye now blurred.

2/The hardest part of a pituitary MRI is deciding if there is cavernous sinus invasion. It makes sense that the more lateral a tumor goes on MRI, the more likely it has invaded the sinus—bc it is going the direction of the sinus. But how far is far enough?

3/This is important bc each time a radiologist makes a call on imaging, they make a bet & they are betting their credibility. And unlike other bets, there is only 1 wager—all in! So it is important to not call it when you might be wrong, bc overcalls destroy credibility.

Don’t forget about adrenal insufficiency!! For patients with things like #syncope or #hypotension, it’s easy to forget about cortisol as a cause! While things like dehydration, infection, & other things may be more common overall… AI is #treatable, so make sure you catch it!!

Primary AI = #adrenal glands themselves are the problem. This means #cortisol AND #aldosterone will be low, while #ACTH AND #Renin will be very elevated in attempt to remedy this!
