1\ when I started out in testing it was all about find the bugs and find them fast. Testers where typically another department to engineering. It was pretty much they build it we destroy it.
2\ This is what inspecting for quality is. You look for the issues at the end of the development process. It worked pretty well when you had an army of testers and no way to push out bug fixes other then sending people disks with patches in the post

1) The power gradient creates no ability for learners to speak up against a preceptor. Trainees in specialty programs want to get hired & can't risk pushing back, esp if you are equity deserving. 2/9
2) The culture forces trainees to be submissive to preceptors in clinical settings, you have to follow their lead. If you can't challenge or speak up, you have a psychologically unsafe space & abuse will happen. 3/9

A common misperception about #psychologicalsafety is that it means lowering standards, giving up on #accountability, or "everyone being wrapped in cotton wool."

Clearly, the place to be is the upper right quadrant, where people are not afraid to have the #difficultconversations that bring real progress.

Those conversations flow upward as well as downward, in an atmosphere of respect that ALSO goes both ways.
This has been my periodic reminder re: what #psychologicalsafetyisnot. I've spilled a lot of ink on that subject; there are some sticky misperceptions there that are probably not going away.

But I hope this helps. And it does have an impressive ink-to-insight ratio :)

So, I promised a thread about my unease with misunderstandings/misinterpretations I notice as the term #psychologicalsafety becomes part of the leadership discourse. Just my own wonderings, as a practitioner immersed in PhD research rn, nothing more or less. Not right or wrong 1/
This thread is not a critique of current research (that might be another one!). It's more about leadership practice implications, & it addresses team PS as the level at which it's understood to be most significant (though more research is now happening at indiv. & org. levels) 2/
The dominant narrative sees leadership as being abt individuals who possess special traits/competencies/behaviours that inspire others to action. It's role-oriented. So we're primed to think about any workplace phenomenon as being under the control of individual leaders 3/

I started with a confession, that the success of the #chaosengineering initiative in your company has less to do with the tools you pick than you may realise... And as the co-founder of the @chaostoolkit, that's quite the hard confession to make!

And, as seen in Maslow’s Hierarchy of Needs, our students’ basic needs for shelter and food are well met along with their need to feel physically safe. However, safety is more than just physical.
According to Harvard Professor Amy Edmondson, “Psychological safety is the belief that one will not be punished or humiliated for speaking up with ideas, questions, concerns or mistakes."

Spending my morning in lockdown with @AdamMGrant @growthfaculty on learning to rethink. Think like a scientist - surround yourself with those who challenge our thought processes and continue to examine your hypotheses based on evidence. Follow this 🧵 for some key learnings!
Create a #challengenetwork of disagreeable givers - tough love that leads us to question ourselves and think again. It’s not enough to ask people for criticism. We need to create #psychologicalsafety for others to challenge you by showing you are open to feedback.
Encourage people to raise problems even if they don’t know solutions. Praise the canary in the coal mine. “Kill the company” or “problem box” exercise needs to be built into the workplace culture. It overcomes our solution aversion.

This is how organisational culture works. Something underneath the surface (not explicit or visible to individual actors) quietly amplifies conformity and dampens outliers:
This is of course, a paradox.

It’s a good thing when it comes to mission, vision, and rejecting toxic behaviour.

It’s a bad thing if it means conforming thinking, seeking “cultural fit” and making it unsafe to challenge the status quo or hierarchy.
Amplifying conformity can be downright dangerous if it means risks are dampened or rejected/hidden. Being able to sense and consider how things could go wrong is crucial to survival.

As a supervisor, you need to make clear to your learners (1) the right answer is: “I forgot to check them”

And (2) your reaction will be: “No problem. Let’s check them now.”

This framework has been applied to reducing harm, mostly through medication error (though intended to achieve population health)

Then was expanded to patient experience

Also, cost of care

More recently we have added clinician wellbeing

The acute care side of the hospital - money maker - thinks of this as type of safety & #quality

Not surprised 2hear that @AmyCEdmondson “stumbled into psychological safety while studying learning organizations.”

Perhaps that’s b/c psychosocial safety is a basic human need that must be satisfied to fully engage w/ learning
“I found that better teams were making more mistakes. Or maybe they were just better at talking about them” @AmyCEdmondson

But there are numerous barriers to doing so, which are called intrapersonal risks. These risks keep us quiet and disengaged and drive impression management
“We choose self-protection over growth.”

In medicine, why do we feel such pressure to protect our egos? B/c they are constantly under duress?
