These naive little med students & personal trainer PhDs don’t have to call families of patients who die from diabetic complications like sepsis, MI

They have never seen an amputation

If they had they‘d likely change their tune about their oreo &poptart inclusionism

These people often med students, dietitians and trainers...
who come on twitter, bombastically call for higher forms of evidence, calling people charlatans

& yet, they never truly observed the system fail. They dont even understand the problem. No experience.

If you want regurgitated vegan agenda, detox plans, supplements, or a fairy tale approach to macros or some nonsense/koombaya feeling of food inclusionism... you’ve come to the right place... twitter (im looking at you #rdchat ), not that the LC echo chamber is much better

Stop saying that higher protein intakes cause renal failure, you are wrong. From the WHO/FAO:
“…the suggestion that the decline of glomerular filtration rate that occurs with advancing age in healthy subjects can be attenuated by reducing the protein content
in the diet has no foundation.”
From the IOM in setting the last DRI:
“…protein content of diet is not related to progressive decline in kidney function with age.”

A causal link? No evidence:

Lots of talk happening on different enteral feeding methods. Here is a thread to describe each and their ideal usages. #RDChat #WhatRDsDl 1/
Continuous (pump assisted): requires feeding bag (feed&flush bag or RTH allows both to be given via pump) & pump. Typically used for ICU, j-tubes or G who🙅🏻‍♀️ tolerate bolus. Rate ordered is infused over the full hour (eg 60mL/hr infused 60mL over 1 hour) 2/ #WhatRDsDo #RDChat
Keep in mind that any volume of formula via pump is given over the entire hour, whereas a flush via pump is usually given in one quick bolus. This can sometimes lead to cramping, dumping esp in j tube patients. Smaller, more frequent flushes are helpful. 3/#WhatRDsDo #RDChat

Folks keep citing this BMJ analysis to push for #vitaminD supps right now. It's not unreasonable to think about VitD but there's very imprecise language going around that "Vitamin D deficiency" is very widespread. This is hotly debated .bmj.com/content/356/bm…
Most folks are citing that many individuals are below 30ng/mL which is the threshold that @TheEndoSociety rec'd in 2011 following the 2010 @NASEM_Health report. There aren't trials treating folks with low -> 30+ng/mL to support the rec,it's mostly epi work and basic bio rationale
The BMJ paper comes in cause folks are saying that it supports a role for VitaminD suppvto reduce respiratory tract infections and may be useful against corona, and there's a high prevalence of VitD deficiency in the population. Makes sense?

1/ The #HAES and @WtMgmtDPG debate at #FNCE today impacted me more than I realized, so please allow me to share what has been bouncing around inside my head for the last few hours - Another perspective that may have been missed.
2/ This "debate" seems multi-layered and hard to pin down, but one of the most controversial points, as far as I understand it, is to argue which of the following is worse - the state of being obese, or the stigma that can be attached to it? #FNCE
3/ When I dare venture to ask why we can't respond to this stigma by teaching clients to internalize things differently, I get dismissed or told things like "You're missing the bigger picture of injustice", or, "that's just your privilege talking." #FNCE
