This just out: Physicians do not logically/correctly estimate the probability of outcomes resulting from sequences of events - a thread. #conjunctionfallacy #probability #medicaldecisionmaking #clinicalreasoning #innumeracy #numeracy jamanetwork.com/journals/jaman… @JAMANetworkOpen
In this article, we showed that physicians estimated the probability of two events both occurring as *more* likely than one or both of the individual events. This is logically impossible and consistent with the #conjunctionfallacy. This #bias can lead to catastrophic outcomes
The impetus for the study was an #OBGYN catastrophe that resulted, in part, from this bias. A woman presented in labor w brow presentation. For successful vaginal delivery, 2 events must happen: reversion to deliverable position, and vaginal delivery w/o #caesareansection

@Zepaks 2/ Na początek kilka informacji:
- podjąłem się analizy jako challenge mema @dingoromuald, którego komentarz (2 mld PLN EBITDA'23) vs. MCap (ok. 1 mld PLN) wydał mi się na tyle absurdalny, że postanowiłem sam to sprawdzić;
- analiza jest wyrazem mojego własnego postrzegania spraw
@Zepaks @DingoRomuald 3/ - nie gwarantuję, że jest ona wolna od wad i błędów, choć starałem się dołożyć staranności, aby konkluzje były oparte na danych, faktach i racjonalnych założeniach;
- jestem otwarty na ich krytykę i dyskusję w temacie;
- jednocześnie zachęcam do własnego researchu i analizy!

Another #sensitivity / #specificity thread.
People are mixing up analytical sens/spec with clinical sens/spec. And translate "clinical" sens/spec in a lab-setting to real-world.
This has implications for the discussions we have about the accuracy of a test.
A #dta 🧵
Analytical sensitivity is how much (or how little) of a certain compound can be measured.
E.g. in blood, what is the lowest glucose concentration or lowest concentration of antibodies to be detected?

Analytical specificity is about the absence of cross-reactivity.
E.g. Does my/
Influenza - antigen test remain negative in samples with other viruses?

This does translate, but is not similar to diagnostic accuracy, as assessed in a lab.

If I have 100 samples of which I am certain that they contain virus particles, how many of those test positive?

Do you know all of these nuances of #pleuraldisease? Why is an #empyema distinct from a complicated #parapneumoniceffusion? What is #contarinisyndrome #explosivepleuritis #chyliform #pseudochylous? Are all #chylothorax milky? #pccm #pulmcrit #pulmonary
#pleuraldisease is fascinating and nuanced. First step is #lightscriteria which give favor to finding #exudates which tend to be more urgent. You only need one criterion, which maximizes #sensitivity. Meaningless statement: "It's an exudate only by protein." One criterion=exudate
(Like on Tinder, the more criteria you require, the more you narrow your pool, compromising sensitivity for specificity.) Transudates I will skip over. Most common #exudate is #parapneumoniceffusion #PPE. If there is or is likely to be #pneumonia, it's PPE. Next task is to...

1/n: On the occasion of current discussions on testing strategies in #COVID19 let's make a short tweetorial on medical decision making (a branch of biomedical cybernetics, which is underestimated but has enormous impact). #cybernetics #biocybernetics #DecisionTheory
2/n: #Sensitivity, also known as "true positive rate" (TPR) is the fraction of the number of diseased subjects with positive test divided by all diseased patients. Since in both numerator and denominator are pure counts it is independent of the prevalence of the disease:
3/n: Likewise, the true-negative rate (TNR), the ratio of true negatives to the sum of false positives and true negatives, is independent of the prevalence:

One of the BIGGEST advantages of having studied in the US, specially at an Ivy League school in a city like Philadelphia is the exposure I received to 2 ethnicities:

Judaism and African Americans.
The FIRST thing I was educated on by my friends at Penn Dental was what NOT to say to and about them. And I wish to share that here as I saw a tweet recently break one of the rules.

Judaism: Any one who has studied history knows who Hitler, also known as "The Fuhrer" was.
He basically is responsible for massacre for millions of people purely because they belonged to the Jewish faith.

What he has done, rather had done, is TERRIBLE and almost nothing can come close to it in terms of cruelty, insensitivity, racial bias and inhumanity.

Closing the loop on #Diagnostics #Tweetorial with example #AppleWatch #AFib #Screening. From a company website: "Atrial fibrillation is a silent killer. The heart arrhythmia causes more life-threatening strokes than any other chronic condition, and will affect 1 in 4 of us."
"But the sad fact is that atrial fibrillation often goes unnoticed: It is estimated that 40% of those who experience the heart condition are completely unaware of it."
Using #AppleWatch technology and #DeepLearning #AI #ML, a device algorithm can reportedly detect atrial fibrillation with high accuracy (c-statistic 93%).
