OK, I'm boing to break this down of why I think we're letting patients down while probably boosting profits for vendors. And yes I'm a bit worked up. A 🧵 1/
First, why do we care? B/c the promise of #TEFCA is that as an individual I can use an app and search for my data across all of my docs and not have to know their name or my username/password. MUCH better than we have today for patients and the whole point of a network. 2/
For individual access, patients have to be ID proofed to NIST IAL 2 by a certified CSP. All very reasonable and a much higher bar (in a good way) than we have today. 3/

There are two perspectives on the IHE SOAP-based specifications (primarily XDS, XCA and XCPD), not mutually exclusive

a) They have worked in practice, so should not be ripped and replaced
b) They are legacy technology and we should work on more modern alternatives.

A 3rd perspective, enshrined in #TEFCA #QTF is:
c) We should continue to make them the core of our health infrastructure

The world "legacy" makes people mad -- my perspective is that technology moves on regardless of whether we like it or not. Some standards, particularly low level networking standards, have stood the test of time, some have... not
