Word “racism” silenced, actively, by #MedEd #MedTwitter #healthcare#professionalism” & “communication” labels

Saying “racism” offends or “hurts feelings” > actual #BLM deaths

If #professionalism itself is racist -makes sense that word suppressed
@FutureDocs
What @ayshakhoury experienced pushes out & silences many

If the editors of @JAMA_current resist hearing word “structural racism” then they certainly are not publishing papers using that word

#MedTwitter influencers show hypocrisy (advice vs actions on masks). Agree:

We live in an attention #economy & what #Tweetiatricians have known for decades (#Twitter started in 2006): importance of drawing attention to factual science-based information as compared to #misinformation & #disinformation, especially on #vaccines

econreview.berkeley.edu/paying-attenti…
You have to be fast & prolific to play catch up to this. I have 77K tweets, a moderate sized band of 6K followers interested enough to tolerate my volume but use of hashtags allows reach across Twittersphere. Trending hashtags = better for riding a way for that attention economy

This is why I do long threads on racism in child & education spaces (child education up to #MedEd) and on the role of women to “protect” only some kids, not others. There is focus on police without looking at the full picture of sources of harm to children of color.
It is very very easy to focus on the Boston police Union leader

but who is it that..

helps him get access to children?
stays silent?
silences abused children?
promotes him?
seeks to please him (be promoted by him)?
sees him as a “good leader”?
softens his image?
protects him?
The role of men & women who police/silence those who would speak up... who prevent identifying the abuser

=extremely important

it is all on one spectrum: if you experience being denied food by a teacher as a 3-year old, will you speak up on abuse...

vs Stockholm Syndrome?

Why is someone who likely does not menstruate a sudden expert on #womenshealth to identify a WOC seeking #data #quality & #integrity as the threat...to women?
"Respecting" social science = using respectable methods, not online surveys.
What you see in the tweet and the "study" is ego, defensiveness & flagrant Dunning-Kruger effect for #womenshealth, #menstrualhealth, #COVID19, #vaccine #research.

The best way to demonstrate the value of a field is to be consistent on #quality not

Academic language on #online #survey #data:
"systematically assess carelessness in a crowdsourced sample" Denial that quick & dirty is poor #quality is an odd use of time & ineffective. One could better defend one's merit, by delivering #quality per best practices and standards.

#MedTwitter showed me the dark side of this, of all times, during a #pandemic. Turns out some who are employed in supposed #SciComm actually stalk patients on behalf of doctors & administrators. Sadly, #bullying patients has gone from "just" in hospitals to dark web.
There is a type of doctor, not all, who believes self a "victim" of patients.. "I am a victim" is used to do mental gymnastics to "justify" #bullying patients who seek answers, report error or harm, initiate lawsuits.

Yes, there is too much litigation. But this is America.
#Tweetiatrician training is for #advocacy on #vaccines #VaccinesWork #VaccinesSaveLives

Recently @JAMA_current published an OpEd about #SoMe harassment, framing it as a problem women experience

It fails to acknowledge the danger/assault/harassment Black boys and men face from white women like this, in person or online
Same for LGBTQ men

Women not the only victims
American feminism and the role of white women & damsel in distress in relation to Black men in America

has a deep and ugly history

that keeps popping up in current affairs, like Amy Cooper in Central Park calling 911 on a birdwatching Black man asking her to respect rules
@JAMA_current’s failure to frame this as an inherently intersectional issue has multiple implications for #SciComm at a time we need to be using correct frames.

First gender is not binary.

An opening line of women vs men excludes spectrum of #LGBTQ & or experience of Black men. Image

Dear #MedTwitter please, please, if you are a licensed medical professional and/or in school to be, do NOT delete your tweets or comply with anyone telling you to do so. It will harm you far more than leaving them up and explaining. Nothing is ever deleted. Can be used in court.
When I was at Harvard Med School and at an esteemed children's hospital, a case against an esteemed professor, that NO lawyer would touch cuz this was someone who had WRITTEN the textbooks and trained every expert, was won ..based on "a document was deleted" - creating suspicion
"Sunlight is the best disinfectant" you will hear me say OVER and OVER - wins & losses *should* be on data/standards but unfortunately wins & losses are in the court of public opinion. So be reliable, credible, and trustworthy. Transparency is one way to achieve that reputation.

Just when I had the #COVID19 #quarantine guidelines down pat...the @CDCgov goes and changes them!

Here’s the quick and dirty on the new guidelines (in 10 seconds) 👇🏼

Read thread for more info.

@OHPediatricians #tweetiatrician @AmerAcadPeds
Why change?
Per @CDCgov the change was an effort to reduce economic hardship for those unable to work during quarantine as well as reduce burden on #PublicHealth system.

#quarantineguidelines #quarantine #COVID19
To be CLEAR: the recommendation remains that a 14 day #quarantine after #COVID19 exposure is still the safest option.

ALSO - local #PublicHealth authorities have the FINAL SAY in when quarantine can end.

@sinanaral on false news

#tweetiatrician peeps, @AmerAcadPeds, @AmerMedicalAssn and @PublicHealth @DrTomFrieden @DrPaulOffit @SaadOmer3 @ASlavitt this is a must watch and Prof Aral’s work is so critical.

People>bots are source of misinformation spread

Fake news spreads as it invokes surprise & disgust. It is also tied to novelty.

Being negative & relying on creating anger & disgust “wins” over positivity. *sigh*

@AmerAcadPeds #tweetiatrician

I’ve long had a concern over the doctors & #Scicomm who “review” “fake news” out there. Yes content may generate anger & disgust among pro-science followers but how often do you boost the stats of the original fake content?

Have also seen sharing of false info about #STEM peers

Hmm..so have to say, the most misogyny I have encountered is on #MedTwitter mostly from anon account doctors or anti-vaxx.

& tho I repost "The Female Lead" I have rare to no trolls or harassers. I have found @LinkedIn pretty female-friendly, in fact. Rarely get a weird message
My first run in was an all male group of doctors discussing IVF and ridiculing acupuncture as not evidence based but they had fragility to any aspect of IVF itself at all questioned or perceiving even tolerance of acupuncture as "quackery"
My point then was more a people in glass houses should not throw stones. There is a difference between clinical trials existing and #EBM as I know from having been on the payer side where business-minded clinicians' narrative advances ahead of science: ncbi.nlm.nih.gov/pmc/articles/P…

Agree. There is a lot more to unpack and there are not simple policy or regulatory fixes. If you think the feds coming down hard on what the population uses to connect and communicate, you’re not a student of history, sociology, psychology, etc.

@sinanaral in @HarvardBiz
I hear all the old, harmful ideas repeated by many who should be at cutting edge of technology. Whereas what @DrvanTilburg describes, if merely digitized to “control” social media, will not work, will harm #SoMe #AI #SciComm #professionalism
From the #healthcare lens there are these potential issues of #AI and #bias including as relates to #COVID19. Yet 30% of business are now using #ArtificialIntelligence in some form. Horse out the barn.

So what is the answer? Ban? Control?

No.

My experiences on #MedTwitter, ostensibly a place for activism & public health - at least as is the approach of a #tweetiatrician

affirmed that

until

we FIX
culture of #medicine
change perverse incentives

until then

#healthcare will continue to fail

even be source of harm
When will we start caring more about problems than petty fights?

When will we spend our energy on #creativity and #problemsolving rather than stalking, blocking, hiding & deleting tweets?

This is not limited to #MedTwitter
It replicates what happens in #hospitals and #academia
Faculty need to step up and keep other faculty in check.

#Safespace is not just a hashtag.

Am grateful I trained under Dr. Ralph Feigin @bcmhouston. He fiercely defended trainees. There are important ways he created safe spaces.

This, today, would be considered flat out torture

First, women’s health was not studied. Even when women’s health was studied, it was by harming black & indigenous women

When women talk of safe spaces I look at race

We know this has existed throughout history ...and now
In modern society, how does a woman with education and privilege seeking career success “lean in”?

She likely has someone else - often a low wage dark skinned woman - looking after her kids, cleaning bathrooms, cooking meals.

What I have come to realize at midcareer & reading history more deeply

privileged women’s advancement requires exploitation of the disadvantaged, especially low income women

It is partly why election results in 2016 *was* voting for own interest in fact
