We shouldn’t be too fixated on vaccine efficacy. What ultimately is important (& I suspect most of us are talking about) is vaccine effectiveness (VE).

Vaccine efficacy is % of reduction of disease in a vaccinated vs unvaccinated group in a trial.

VE is the real world impact.
V.efficacy measures individual protection in a controlled environment while VE looks at how protected is a population after vaccination. There is a whole host of factors influencing it.

And not necessarily v.efficacy is directly proportional to VE. Image
Focusing on only efficacy will distract us from implementing a good vaccination plan to maximise effectiveness.

Factors like administration, cold chain, coverage (which impacts herd immunity) all can greatly increase effectiveness, which is what we want.

1/USA #VaccineStrategy improving

💉 600k+ shots in arms today
📈 2.7% vaccinated
😷~100-125m have had Covid (estimate)
---
~135m OTOS ("on the other side" -- my estimate)

With 1m infected & 1m vaccinated a day = 2m OTOS a day

~Feb 15th we will have 50%+ of the USA "OTOS" ImageImage
NY & California, the worst-performing states, announce less precious vaccine deployments. 24-hour, @friedberg-style rollouts coming soon.

If @NYGovCuomo had widened eligibility first, as in: nytimes.com/2021/01/08/nyr…
Note that this happened on January 8th, 2021.

(1/n)
then NY Hospitals would have acted differently to his prior order of "Use it or Lose it, and Get Fined", issued on January 4, 2021. At this time, hospitals were NOT allowed to vaccinate patients, even though many were clamoring to do so. (2/n)

As a consequence, it is not surprising (and entirely predictable based on the incentive created by @NYGovCuomo) that many academic medical centers vaccinated lower-risk and non-patient facing staff, as outlined today in the @nytimes by @apoorva_nyc (3/n) nytimes.com/video/us/polit…

Important for those living with #HIV. As it currently stands, if you have a #CD4 over 200 and no other health conditions it is expected you will fall into phase 6 for getting the #covid #vaccine.
However, the #vaccine is not currently expected to be given through #HIV clinics (although this may change). This means that if you have not shared your status with your GP it is likely that you will not be considered for the #covidvaccine until later phases!
This has been raised and being looked at by @THTorguk @NAT_AIDS_Trust @DHSCgovuk @NHSEngland

It is fantastic to see the #COVID19 vaccination programme begin #VaccinesSaveLives. Some initial thoughts for now & the weeks/months ahead (personal views): 1. All hands on deck approach in hospitals led to efficient systems vaccinating large numbers in a short space of time
2. True multidisciplinary working meant that expertise to solve problems & evolve the process was on hand 3. The hospital facilities that were used for this will have ~ 2 week lull before the second dose (accepting some delayed vaccines in between)
4. Mass vaccination centres won’t be set up over night so these suggestions focus on the next few weeks 5. Legislation is in place enabling a broad range of health professionals to vaccinate

If you’re worried about the delayed second dose of the COVID vaccines here’s a thread.

I live with my wonderful mother-in-law (below): she is high risk and has had a single dose. She's hoping for another but is worried about a delay. You may feel the same...

I’ve also received a single dose of the Pfizer vaccine and since COVID gave my identical twin brother a serious heart problem I have a dog in the fight (two dogs including the MiL). I don’t want this virus and I have to go to work in a hospital where there is a lot of it about.
Overall the decision to extend the schedule so there is a 12 week gap between doses should save more lives. Now I’m a nice guy but of course I care more about MY life (and my mother in law's!) than just any old life like yours.

People need to understand that this Covid Vaccines ain't a cure not a solution. Then people need to understand that these people don't care about you..Look at who funds the Vaccines... Bill Gates is one of them.

Secondly, You need to look at the game happening where we are in a space where Political leaders, special interests seem to know more than 'Doctors etc'

Where Doctors are called 'Conspiracy theorists & Bill Gates is correct guy 👀

We have always said the second wave is going to be coming and the only way we will be protected is by developing herd immunity through vaccination. #VaccineStrategy
The only way of being able to deal with the #COVID19 pandemic is the provision of immunity through vaccination. We have been trying to follow what the best way is to deal with this. #VaccineStrategy
The vaccines will need to be made available quickly so that most of our citizens are
covered by the end of the first year of rollout - this year. #VaccineStrategy

Concerned about the #COVID19 vaccines and autoimmune disease?

Both the #Moderna and #Pfizer vaccines do not contain an attenuated virus or instructions (via mRNA) to make viral particles that could replicate in vaccine recipients. (1/n)
I have psoriatic arthritis (on #methotrexate for about a decade) and had no hesitation to receive the vaccine @sloan_kettering. I am fortunate and was able to stop methotrexate for one month prior - this is atypical for most patients with autoimmune diseases - and will (2/n)
resume the medication a month after the second shot. I stress that this is not a medical recommendation for other patients with psoriatic arthritis or any other autoimmune disease. Patients should discuss how treatments for autoimmune conditions may impact vaccine immunity (3/n)
