@alpanashuklamd takes the stage at the @ColumbiaMed @WeillCornell #Obesity Conference Image
@alpanashuklamd et al examined the effect of macronutrient sequencing (carbs first vs carbs last) in ppl with diabetes, prediabetes, or gestational diabetes

This study looked at those with prediabetes:
dom-pubs.onlinelibrary.wiley.com/doi/abs/10.111…

2/
Among those with prediabetes, we saw significant differences in glucose, insulin, ghrelin, and GLP-1 -- all favoring the "carbohydrates-last" food order

SO EXCITED FOR THIS

Dr. Rudy Leibel of @ColumbiaMed, everyone Image
Key quote:
"The RISK of developing obesity comes from genes."

Note that he doesn't say obesity itself is caused by genes. This would be an oversimplification!

You've seen this graph in several versions. This is what happens when genes set a risk level and the environment selects for it, resulting in a specific phenotype

Sugar consumption and its ill-effects on health

1. Recently, a review article was published that looked at the association between sugar consumption & various diseases.
Data was collected from 8601 unique research papers published earlier on this topic. doi.org/10.1136/bmj-20…
2. Highest versus lowest dietary sugar consumption was associated with increased body #weight (sugar sweetened beverages) & fat accumulation in liver.
*Each serving/week increment of sugar sweetened beverage consumption was associated with a 4% higher risk of gout.
#MedTwitter
3. Each 250 mL/day increment of sugar sweetened beverage consumption was associated with a 17% higher risk of coronary heart disease & 4% higher risk of all cause death.
Every 25 g/day increment of fructose consumption was associated with a 22% higher risk of pancreatic #Cancer.

#obesity brain atrophy is similar to #Alzheimers disease (AD)

1. Obesity is linked to multiple health hazards such as diabetes, hypertension, cancer, heart attack and stroke.
Recently, excess #weight has also been related to brain atrophy and cognitive decline.
#MedTwitter
2. Obesity-related grey matter atrophy resembles that of AD as per a recent research.
Cortical thickness was reduced in frontal, parietal & temporal regions.
The similarity in brain atrophy between AD & obesity was the highest in the right temporal and left prefrontal cortex.
3. Conclusion
Excess weight management could lead to improved health outcomes, slow down cognitive decline in aging, and lower the risk for Alzheimer's disease

A very interesting thread on @Reddit with @Walgreens employees and others discussing the rules made by @LillyPad to only place through prescriptions with the diagnosis of #diabetes - #Mounjaro is NOT YET indicated by the @US_FDA for #obesity as a diagnosis
The stigma for #obesity is directed at those with the disease. Unfortunately our society perceives that obesity is a “second class” disease and should be treated as such. Here @Walgreens employees are admitting to discriminating against patients 1/
The patients have been prescribed #Mounjaro “off-label” for the treatment of #obesity (because this medication has been shown to result in the highest weight loss of any medication at 22% total body weight lost) 2/

Food myth busted: Potato is not bad for diabetics

1. Potato has got a bad name, and diabetics are advised to avoid it because of its high glycemic index. However, findings of a recent research busts this myth.

#nutrition #healthyfood #diet #MedTwitter #diabetes #weightloss
2. This was an 8-week randomized, controlled feeding study comparing Potato and Bean diets (50-55% carbohydrate, 30-35% fat, 15-20% protein).
Thirty-six participants (18-60 years, BMI: 25-40 kg/m2) with insulin resistance were enrolled.
Ref: doi.org/10.1089/jmf.20…
3. Among study completers, HOMA-Insulin resistance in the bean group showed an average decrease of 1.4 from baseline (P = .02 ); a similar decrease of 1.3 occurred in the potato group (P < .05) with no significant difference between the two diets.

Though not its intention, this study from #OW2022 illustrated the concepts of satiation vs satiety very well

Colloquially we use these terms interchangeable

But they are technically different

#obesity #science #research #physiology #pathophysiology
#weight

1/ Image
Satiation is the feeling of fullness WITHIN a meal while satiety is the feeling of fullness IN BETWEEN meals

Even though current meds like GLP1s seem to affect both, it’s an impt distinction especially when some peptides (eg, amylin)
may only effect satiation (⬇️meal size) and not satiety (change meal frequency)

[btw This study was an intracerebral injection of calcitonin into male rats and calcitonin is an agonist at the amylin receptor in the ventral tegmental area]

Milestone: My 2nd #runniversary and 10000 Km

1. Nov 6th 2020 was the most important day in my life, when I took complete control of my health & fitness. From a #workaholic, I converted to workaholic + #runaholic
Here, I present the milestones of my past 2-year #running journey
2. I completed 10,163 km of running in the past two years (6th Nov 2020 to 6th Nov 2022: 731 days), at an average of 13.9 km/day. This included 648 runs of 10k or longer, 110 runs of half marathon (21.1 km) or longer and one marathon (42.2 km).
3. I usually run at a pace of 7:15-7:30 min/km and my best pace is 5:45 to 6:00 min/km. My PB in 10 k is 57:03 min and in HM 2:04:41 hours. I cherish the #Ladakhmarathon HM the most, which I finished in 2:24:33 hours (on 11 Sep 2022).

Awesome October 2021: A month of 31 half #marathons on 31 consecutive days- sharing my experience
1.Starting from 30th September, and ending on 31st October 2021, I completed 32 runs of 21.1 km or longer, with a total mileage of 817 km in 32 days (average of 25.5 km/day).
This included one Full marathon (42.2 km) on 30th October.
2.The purpose was to test my #endurance, and to see the #physical & #mental effects of long distance #running on 32 consecutive days. I present my observations here.
3. How difficult was it? In reality, it was not difficult. Once you get up and start the run, it is easy to complete. The most important is- you need 2.5 to 3.5 hours time to compete the distance. I could manage that in the mornings between 5 and 8:30 AM.

Heart rate during work out: What is desirable and what is unsafe?
1. For an aerobic activity to be effective, the intensity should be at least moderate. The best way to measure the intensity of workout such as running is to check the #Heart rate. This thread is about heart rate.
2. What is resting HR?
Resting HR is the HR noted on waking up after a good night's #sleep before you leave the bed. For most adults, it ranges between 60-100/min. For physically #fit people, it may be lower, and may range between 40-60/minute.
3. Which is better- lower or higher resting HR?
Lower resting HR is better. It means- heart has to contract lesser number of times to do its work. Studies have found that a HIGHER resting heart rate is linked with LOWER physical #fitness, HIGHER blood pressure and body #weight.

It's weekend and it's the end of the month. The #markets are closed.

So there is the perfect time to analyze the #momentum & relative #strength of the markets.

This information is relevant for all #investors, but especially for #trend #followers 📈📈📈

#marketresearch

🧵
The #momentum of the markets are one of the most powerful forces in the evolution of the #asset #prices.

The #reversion to the mean is the opposite force, which is equally powerful. Momentum works on a short and medium term (up to 3 to 5 years).
There is a ton of academic research related to the momentum and the benefits of using it and relative strength. But presenting it this is not the purpose of this #thread.

Relative strength is actually a comparison between the #momentum of two or more #markets.

9 causes of low sperm count/quality you should know

1. Infections
2. Varicocele

4. Drugs

8. Alcohol & smoking

A thread 🧵

Please RT for awareness
Low sperm count or #oligospermia means the semen a man ejaculates during an orgasm contains fewer sperm than normal. Medically, it means having less than 15 million sperm in 1ml of your semen.

If there's no sperm at all in your semen, it is called #azoospermia.
Having low sperm count significantly lowers your chances of being able to impregnate a woman.

Many families have been torn apart because of the resulting infertility.

Unfortunately, in some cases, people erroneously think the cause of the infertility is the woman.

Very disappointing #obesity strategy from the UK government @BorisJohnson.Disappointing because it is:1)#weight biased 2)non #evidence-based and subsequently unlikely to be effective 3)ignores people with #obesity ie ignores treatment of obesity #obsmuk 👇 gov.uk/government/new…
2/The underlying issue that needs to be tackled in any #obesity strategy is bias and discrimination against people with obesity and the attached stigma. Weight bias and stigma hinder the efforts to initiate a proper strategy to fight obesity. #obsmuk #actonobesitynow
3/The measures that the UK government introduced are non evidence based. There is no evidence that the strategies the government introduced will prevent #obesity or treat #obesity. Subsequently, likely waste of time and money.

for those who missed my @aberdeen_ucu talk today on #sugar, #breakfast, #health & #appetite, a wee thread on the fascinating properties of sugar...
full talk (poor sound qual) can be found here:

1/21
#sugar is demonised in the media, literally described as "evil". myths include: its addictive, causes cravings, makes you hungry, makes you overeat, makes you gain weight.

current #publichealth guidelines target reducing sugar
2/21
#breakfast is touted the "most important" meal of the day. what does "important" mean?

breakfast is the highest #sugar meal of the day so a good meal to target to reduce total sugar.

breakfast = complex to define, but broadly breaks the overnight fast (break-fast) 3/21
