The findings of this one suggest that dietary carbohydrate and exercise may generate independent and interactive effects on the plasma metabolome, resulting in the generation of distinct metabolic phenotypes that may influence appetite regulation and perhaps energy intake. Image
- This study aimed to determine the interactive effects of carbohydrate and exercise on the plasma hormonal and metabolite responses and explore potential mediators of exercise-induced changes in appetite and energy intake across nutritional states.
- In a randomised-crossover fashion, 12 male participants completed four study visits that involved the consumption of a control (water) or carbohydrate beverage (75 g maltodextrin, 300 kcal), followed by a 30-minute rest or exercise session (75% V'O2max on a cycle ergometer).

- This is the first umbrella review on sarcopenic obesity conducted to provide comprehensive insights into the effectiveness of different nutrition and exercise interventions for adults.
- Four systematic reviews with between 30 to 225 participants were included in the umbrella review.

In this one, sustained weight loss was associated with lower HbA1c at 10 years and improvement in lipid profile, while maintenance of ≥7% weight loss at 1 year was associated with decreased incidence of diabetic nephropathy at 10 years in patients with diabetes. ImageImage
- The study evaluated 129 patients with diabetes and obesity enrolled in a 12-week translational model of intensive lifestyle intervention.
- At 1 year, participants were divided into group A, who maintained <7% weight loss (n=61, 47.7%), and group B, who maintained ≥7% weight loss (n=67, 52.3%).

Follow-up was continued for 10 years.

In this one, an intervention of 200 minute of walking per day was associated with profound effects on the metabolic profile of the participant, a reduced body weight, similar in magnitude to that seen after bariatric surgery, and increased fatty acid beta-oxidation.
- This was a feasibility study of a subject who performed an exercise intervention of low-intensity, non-fatiguing walking on a deskmill/treadmill for 200 minutes daily, approximately the average time a German spends watching television per day, for 6 days a week, for 4 weeks.
- Data suggested that walking can have a profound effect on the metabolism of a person – an effect that is visible both acutely as well as after 1.5 days.

🧵 How Gila monster’s venom inspired a new fat-loss drug that everyone is talking about.

A story of unlikely collaboration.

This is how Semaglutide (Ozempic) was invented: Image
Semaglutide is an injectable drug that is used to treat Type 2 diabetes.

But, how did a diabetes drug become a potential game-changer for obesity?

It all started in 1995. Image
An endocrinologist named Daniel Drucker learned that the venom of Gila Monster lizard contained hormones that can regulate blood sugar.

His curiosity led to a call with a zoo in Utah & had a lizard shipped to his lab where he started experimenting on the deadly venom. Image

In this one, a phenotype-tailored lifestyle intervention was associated with further improvements in measures of body composition and triglycerides compared with a standard lifestyle intervention.
- This 12-week, single-centre non-randomised proof-of-concept clinical trial included men and women aged 18–65 years with a body mass index (BMI) greater than 30 without history of any bariatric procedure, and current use of any medication known to affect weight.
- Participants were assigned to their intervention based on their period of enrollment.

The findings of this one may suggest that individuals with lower habitual exercise and/or higher fasting concentrations of PYY are more likely to show a compensatory increase in energy intake for energy expended in exercise. Image
- The present study aimed to identify predictors of post-exercise energy intake and compensation in healthy adults following a single 45-min aerobic exercise bout.
- Individuals with lower habitual exercise and/or higher fasting concentrations of PYY ate more after an acute exercise bout, even after accounting for the energy expended during the exercise bout.

This systematic review finds that the literature overall suggests that the metabolizable energy of nuts is lower than that predicted by Atwater factors, something that may in part explain the lack of an effect of nut consumption on body weight reported in the literature.
- This systematic review with narrative synthesis aimed to examine the metabolizable energy of tree nuts and peanuts in vitro and in adult humans.
- Findings suggest that the metabolizable energy of the nut types investigated (almonds, cashews, hazelnuts, pistachios, walnuts, and peanuts) is lower than what would be expected following application of Atwater factors.

In this one in adults with obesity without diabetes who initially lost at least 5% of body weight, only the combination of an aerobic exercise program and a GLP-1 receptor agonist improved postprandial glucose & glucagon responses and beta cell function compared with placebo. Image
- The aim of this study was to investigate glucose tolerance, glucagon response, and beta cell function during a 1-year maintenance period with either exercise, the glucagon-like peptide-1 receptor agonist liraglutide, or the combination after diet-induced weight loss.
- In this randomized placebo-controlled trial, adults with obesity (BMI: 32–43 kg/m2) without diabetes underwent an 8-week low-calorie diet (800 kcal/d) and were randomized to 52 weeks of aerobic exercise, liraglutide 3.0 mg/d, exercise and liraglutide combined, or placebo.

- A reduction in nonexercise physical activity after exercise may reduce the effectiveness of exercise interventions on weight loss in adults with overweight or obesity.
- The purpose of this secondary analysis was to examine the effect of a single bout of aerobic or resistance exercise on nonexercise physical activity and sedentary behavior in inactive adults with overweight or obesity.

Time-restricted eating alone may not result in long-term weight loss

1. Time-restricted eating (TRE), a form of #IntermittentFasting (IF), has gained popularity for #weightloss.
However, weight loss was not seen in a recently published study with TRE.
#obesity #MedTwitter
2. 547 participants (78% women, mean age 51 years, mean BMI 30.8) were followed up for 6 years.
*Mean interval from first to last meal was 11.5 hours and was not associated with weight change.
*The number of #meals per day was positively associated with weight change.
3. Total daily number of large (>1000 calories) and medium meals (500-1000 calories) was associated with increased weight over follow‐up time, while total number of small meals (<500 calories) was associated with decreasing weight.

Contrary to some studies suggesting that time‐restricted eating could improve circadian rhythms and play a role in metabolic regulation, this one found that the frequency and the size, rather than the timing, of meals to be stronger determinants of weight change over time.
- The window of time between first to last meal was not associated with weight change over an average of about 6 years of follow‐up.
- The average daily number of large and medium meals was associated with increased weight over time, suggesting that the meal frequency and meal sizes, rather than the timing of meals, was a stronger determinant of weight gain over time.
