1. Background: An ongoing outbreak of coronavirus disease 2019 (#COVID19) has spread around the world. It is debatable whether asymptomatic COVID-19 virus carriers are contagious.
2. We report here a case of the #asymptomatic patient and present clinical characteristics of 455 contacts, which aims to study the infectivity of asymptomatic carriers. (link at the end)
3. Material and methods: 455 contacts who were exposed to the asymptomatic #COVID19 virus carrier became the subjects of our research. They were divided into three groups: 35 patients, 196 family members and 224 hospital staffs.

1. I had a video appointment with my Dr. today. I asked him about #COVID19 cases - he said there are almost none. I also asked if they are prescribing the #Hydroxychloroquine protocol. He said only for those hospitalized.
2. He said the few positive cases they are seeing are mostly #asymptomatic and do not need anything except to be periodically monitored to make sure they are OK. We joked about how #COVID has cured all other diseases because nobody dies of anything but COVID now.
3. He told me something very disturbing that I bet most people do not know. He said almost every death is being labeled as #COVID because the hospital makes more money - but here is the disturbing part: most life insurance policies have a “#pandemicexclusion”.

Why is #asymptomatic spread important? Well today I was thinking about #polio virus. It was very #contagious, but 95% of people had no symptoms but were still infectious. About 4-5% of people had minor illness: fever, headache, nausea, vomiting, weakness. #SARSCoV2 #COVIDー19
2/ About 1-2% of people had severe muscle pain, and stiffness in the neck and back. <1% of cases resulted in #paralysis including the legs and the nerves controlling breathing and swallowing necessitating use of the iron lung #ventilators. #Polio could attack the #CNS.
3/ At its peak in 1952, more than 21,000 #Americans contracted a paralyzing form of #polio, and 3,000 died from it. Once infected, there was no treatment besides time and tending to the symptoms. Interestingly, polio mostly attacked young children under age 5.

THREAD:
1/ Given my interactions with @ASlavitt and @jeremyfaust yesterday, I decided to make a compiled list of data on asymptomatic transmission. This is not complete, but the most striking data I have seen on #COVID19 and #SARSCoV2 worldwide.
2/ A nice summary published in @TIME yesterday:

Covert coronavirus infections could be seeding new outbreaks

As said in the article, I am keen to engage in a meaningful and constructive dialogue on #COVID19SWEDEN, based on science... so lets explore some of the science in the points raised. svd.se/brittisk-exper… via

1. The role of #asymptomatic and #presymptomatic transmission. As previously Tweeted, there are clear examples of #asymptomatic transmission... including through non-household contact: #communitytransmission nejm.org/doi/10.1056/NE…
2. Here is a pre-print of a review which reports multiple examples: medrxiv.org/content/10.110… (as always, caution with pre-prints, but the take home here is there are several published examples)

Our research team @CUHKMedicine analysed >300 specimens from 14 #HongKong patients confirmed with #COVID19 and discovered that the virus was detectable in the #stool samples of all patients, regardless of the degree of illness.
3 out of 14 patients still had viruses in their #stool samples even though the virus was no longer found in #sputum, nasopharyngeal and deep throat saliva samples.
Our study suggests that virus shedding in stool may have public health implications. First, it may serve as an alternative #screening tool in people without #respiratory symptoms.

1. If there are #asymptomaticcarriers as they reported from the start, I just don't see the logic of not testing everyone. What if there are millions of #asymptomatic cases? Shouldn't we perhaps should do spot checks to determine the size of the asymptomatic population.
2. But until we find out, we have to isolate. That's all we have to fight it but the good news is if we do it we can survive the wave that surely coming.

It would be helpful for some anecdotal reporting on asymptomatic cases. How'd they spot them? @TheLancet @NEJM @CDCemergency
