Agree and...

Many of us also saw an ugly side of ivory tower

multiple out of touch clinicians & non-clinical faculty overpolicing minority, LGBTQ, women clinicians, trainees, students

Revealing what drives distrust of institutions/faculty

Must fix this for effective #SciComm
#SciComm currently lacks Black or marginalized group (including BIPOC, disabled, etc) leadership within it which explains why so called leaders within it cannot recognize effective public health messaging that works in communities of color, marginalized groups.

Need more:
This video shared by @KateWalshCEO of @The_BMC represents a divide in #SciComm & #MedTwitter & #professionalism
Some find clinicians dancing offensive
The thing with diversity is not everyone at an institution is on the same page, evidenced by #Medbikini

This is why I rarely block people, except if flat out and repeatedly abusive or manipulative and not acting in good faith.

Super important to avoid overly curated feeds or echo chambers.

Keep feeding your mind new content. Challenge your assumptions.

I am quicker to block anonymous accounts who engage in ad hominem - latter signals weak argument. Plus, if you dish it out, be ready to take it. If you lack the gonads (of any gender) to put yourself out there, I don't really respect you and have limited interest in your content
I do appreciate people who introduce me to new sources of data or new ways to look at data. Show me articles, evidence, analysis, and we have a conversation.

I don't (try not to) get elitist/snarky like some do to assert dominance/superiority.

I like to learn how others think.

Agree. The system has failed frontliners. Healthcare relies on shaming to control, diminish, shut down. Let’s NOT pass that on to others.

Back in spring when not enuf PPE for clinicians, and CDC & Fauci saying not to mask, I saw clinicians shaming public who WERE wearing masks.
We lose credibility this way. Scientists already are not believed or respected. In fact, are ridiculed by many. Spoke with @ETSshow on how the “snark” culture is a self harming one for communicating in public. #MedTwitter antics reduce credibility.

Much of how scientists act would get you laughed out of a room in business, ignored, lose your seat at the table, not invited back

Blocking, deleting, snarking, shaming

is not how it works among ppl who make high powered deals

Nor does it engage public blogs.scientificamerican.com/observations/d…

As a physician, can say this about doctor culture in the U.S.: we are a socially stunted puerile bunch easily manipulated by sales & marketing ppl or by metrics of competition, obsessed with status, prestige, ego..

...not my doctor friends...but we hang out at the margins.
The high school level drama that plays out in hospitals, then the replica, amplified, on #MedTwitter is 😱 No wonder our profession kills so many patients when we are so busy policing on pettiness & ignoring standards of quality, safety, population health ncbi.nlm.nih.gov/pmc/articles/P…
There are many reasons why I am hanging up the stethoscope, one of which is that I cannot in good faith go into any clinical setting and feel confident about “first do no harm” - The system is too misaligned, not driving quality, safe results

Then, the human dynamics are
😨😣😖

This. Black nurses like @mclemoremr who not only have authentic clinical and community experience, not only know the disparities data, but also are winning grants to do the studies themselves belong on the transition committee or COVID Task Force.
In contrast when I have taught medical students straight from @AmerAcadPeds policy statements on racism in a hospital with a disparities center, have been told by a division chief that nurses find me “too political” which exactly is this that @COCoQC lays out

Not safe spaces Image
Repeatedly I “break ranks” - I don’t spend my time on outrage over “scope creep” - who is serving in the communities where Black women die at a 12x higher rate from maternal mortality? Not the doctors or nurses battling on own job & pay.

I'm always amused by sales or marketing or communication professionals attempt to sell to me how I need their help, how I need their protection, etc or it could be "mutual benefit" to connect. Oh, then what is your rank for what you are paid to do and attempting to charge me for?
Why am I able to outpace, by far, sales, marketing, and communication professionals? What is it they are teaching you wrong in school and/or you are unable to apply for success in real life?

Likely, the answer is: authenticity
Lived experience
Open communication
Values
Yes, metrics matter
Yes, reputation needs to be protected
But as we see happening with Dr. Fauci, those who wish to suppress science-based information (that they find embarrassing or wish to hide) will malign the person speaking about verifiable facts. Blowback does happen.

Show me a doctor-only staffing model that allows us to care for all Americans, including rural and inner city, those on Medicaid, uninsured, undocumented. The system is broken. We need a range of clinicians.
I am fine being unpopular with doctors. I don’t give any clinician a pass, tho. I don’t give any faculty a pass either.

What I ❤️❤️❤️ @MIT @MITSloan @MITSloanFellows is get to be honest, creative, expressive, fail, try again.

Pushing past boundaries is the goal. Have an impact.
We need fewer silos and turf wars. We need to break down walls and boundaries.

Fail often but fail smartly. Expect to fail. Have a disciplined process by which to learn from failure.

Every time you learn you have pushed a boundary of your mind or skills or effectiveness.

Much debate on #SciComm. Little formal guidance exists. Those on academic committees that oversee ethics & professionalism are unfamiliar with social media. And now we are in a crisis and #pandemic of many levels. #publichealth very different from industry (1st vs rest).
In #publichealth one, the 1st points are be first & be right

There is an inherent tension between these two

With disrupted public health coordination, like cutting off from WHO, changing guidance from CDC (masks only for clinicians, no everyone wear them) how to be accurate?
In comparison, industry guidance #SoMe guidance. Industry has a lot more experience with this and uses it for marketing, market share, customer outreach, customer brand recognition, brand management. The focus is prestige, optics, reputation - there are monetizable traits.

Love, LOVE @NicoleB_MD for doing this. Shout out to @DrToddWo & @DrJCoftheDC too for their savvy outreach to youth patients- am especially impressed with @DrJCoftheDC of @OHSUNews as a role model in communities of color. @AmerAcadPeds #TikTok #tweetiatrician #Pediatrics
*This* is what it means to create “safe space” - no shaming or judgement, only encouragement, kindness, affirmation from a #tweetiatrician who shares that she too was a #teenmom.
Meet patients where they are.
Be a role model.

Am learning so much from @DrJCoftheDC of @OHSUNews - on engaging youth through new forms of #socialmedia like #TikTok and being a role model for #BlackExcellence here in partnership with @NFL players @NFLPA
