Under #BluePremier in 2019, @BlueCrossNC’s value-based care program, providers across 5 health systems & practices affiliated w/ @AledadeACO served 536,000 members.
These health care providers also generated an estimated $153 million through cost savings, quality improvements and slowing the rate of spending on health care. 👏👏

Through our shared risk financial model, a Blue Premier health system shares in cost savings if it meets goals to improve the health of patients – and shares in the losses if it falls short of targets established by the model.
And we’re making rapid progress. We are on track to have 50% of members served by providers in shared risk value-based agreements next year. To hit this goal, we have three focuses – health systems, independent primary care, and advanced primary care.
Another long, technical thread to end the day! @BlueCrossNC is on a mission to transform health care. This means sharing how we tackled problems that have stymied efforts at payment reform at scale, including how to fund the inclusion of self-insured segments within #BluePremier.
Fully insured segments are straightforward – as a plan, we can accrue the estimate reserves that we track on an ongoing basis for any given risk arrangement in which we hold the dollars. But self-insured populations are a different story. Many plans have faced this quandary.
Some have just left them out of the included populations entirely. But leaving out these populations dampens the value of the program and shortchanges the members who will benefit from accountability and alignment of providers to improve outcomes, experience, and affordability.
A critical piece of #BluePremier is requiring enhanced sharing of data between @BlueCrossNC and our provider partners. Shared data & new analytic capabilities enable us to hold all parties accountable to the highest standards of care and deliver better member experiences .

There’s a lot of hype around the promise of innovation, especially in data. But innovation requires alignment across stakeholders in the health care system to want the same thing. @BlueCrossNC has taken the first critical step to jumpstarting innovation by paying for value.

We are working with @AledadeACO @Farzad_MD to accelerate the success of these independent practices in accountability for their patients. Aledade will arm practices with workflow technology, analytic insights, and risk mitigation. /3
Even as we make this #BluePremier announcement, we recognize that the hardest work begins now – that is, changing the way that we deliver #healthcare to improve its quality and make it more efficient.
We pledge to walk with the frontline doctors, nurses, NPs and PAs who provide care to our members and all North Carolinians through this journey.

We’ll use risk adjusted, trended, historical baseline to determine shared savings/losses for the attributed population. In two-sided risk, we’ll apply a discount to ensure the plan and its ASO customers have savings even as we pass back higher amounts of savings. #HealthData /2
This fits our first lesson: meet providers where they are—try to move too fast and lose opportunity. Given this is our first broad scale total cost of care model with many of these organizations, need to start gradually. /3
