🔗 jamanetwork.com/journals/jama/…

This 🧵 will explain the study in detail /11

β-lactams (e.g., #meropenem), are typically given intermittently, but continuous administration can provide constant serum levels and may improve outcomes.

In fact, several meta-analyses reported reduced mortality when administered continuously. Image

Therefore, we conducted a multinational, double-blind, randomized controlled trial to test the hypothesis that continuous administration of #meropenem, compared with intermittent administration, would improve clinically relevant outcomes in critically ill pts with #sepsis Image

The @PATCHTrial, led by @RussellGruen, a study of pre-hospital TXA for severe trauma in adults at high risk of acute traumatic coagulopathy treated in advanced trauma systems, has been published today in @NEJM. #CCR23 Here’s a summary of what we did...
nejm.org/doi/full/10.10…
The effect of in-hospital TXA in trauma was evaluated in the Clinical Randomisation of an Antifibrinolytic in Significant Hemorrhage (CRASH)-2 trial. TXA,
administered within 3 hours of injury, was reported to reduce 28-day mortality

➡️ There are ~ 5.8 million trauma deaths per year globally
➡️ Trauma is the largest cause of death below the age of 50
➡️ One-third of severely injured trauma patients are coagulopathic at hospital arrival
➡️ Fibrinogen is often used, but is it effective?

a short 🧵

1/10 Image
➡️ Trauma-induced coagulopathy is due to inflammation & shock
➡️ It is worsened by hypothermia, acidosis, and hemodilution
➡️ Coagulopathy due to hemorrhage is strongly associated with mortality
➡️ 1/3 of early trauma deaths are due to uncontrolled haemorrhage

➡️ Fibrinogen is depleted early during major bleeding
➡️ It can be replaced with fresh frozen plasma, cryoprecipitate or fibrinogen concentrate
