Microbiota-based therapeutics in the treatment of recurrent Clostridioides difficile 🦠

@DrPaulGastro @Khanna_S @caterina_oneto

#MedTwitter #GITwitter #IDTwitter #ACG2022 #Cdiff
Pathogenesis of CDI
1. Disruption of #microbiome (antibiotics) ▶️ loss of colonization resistance
2. Colonization
3. Spore ▶️ vegetative ▶️ toxin
4. Recurrence cycle or restoration of microbiota
Antibiotics control acute disease, but do not protect against recurrence

Or #Cdiff pearls

Or how I approach C. diff in light of new guidelines
PCR tests are very sensitive but specificity stinks

Be sure of appropriate clinical context (true diarrhea, diarrhea not due to another cause) before testing.
Initial tx with PO Vanco. Guidelines prefer fidaxomicin but too expensive. Reserve fidax for highest risk (SOT, BMT).

Does oral vanco ppx during systemic abx therapy prevent healthcare facility-onset C. diff infection in high-risk patients?

@MannyGuajardoMD recently reviewed a study on this very topic at our virtual JC.

1/ While on my current GI rotation, I've been reading about Clostridioides difficile because, you know, #IDNerd. I ran across something I had not learned about before:

A Hypervirulent Strain of CDiff!

So naturally, I had to look into this.

/thread #Tweetorial #CDiff #IDTwitter
2/
What is the name of the strain of hypervirulent CDiff?
3/
The hypervirulent strain of CDiff is known as NAP1/B1/027, which stands for North American pulsed-field gel electrophoresis type 1, restriction endonuclease analysis type B1, PCR ribotype O27.

That's a mouthful.

So what's the big deal about this strain?

Round 2 of the Great ID Debates of #ASHP18 @ASHPOfficial comes to us from the brilliant minds of @SIDPharm members @TimbrookTT and @julie_justo. To start us off, Tristan makes the case for #fidaxomicin (FID) for treatment of Clostridioides difficile (#cdiff) infection
.@TimbrookTT sets up the major points for FID in the treatment of #Cdiff: 1- ⬇️recurrence compared to #vancomycin (VAN) in RCTs + ⬆️in cure without recurrence (aka global clinical cure) 2- this held up in real-world data 3- cost-effectiveness analyses support 1st line use #ASHP18
.@TimbrookTT notes the 2018 @IDSAInfo guidelines for #cdiff (idsociety.org/practice-guide…) recommend FID equally to VAN and for first recurrence, quality of evidence is higher 👀🤔#ASHP18
