I am a recent but huge fan of @Freakonomics podcast. I've also been enjoying @DrBapuPod by @AnupamBJena

Episode today was about #ArtificialIntelligence in #medicine and I want to share my concerns about the discussion on racial disparities in knee pain (starting at 19:00)

A 🧵
#1 - The suggestion that only pain that can be explained by pathology seen on a knee xray is "real, organic, genuine" and other pain is "in someone's head" is hugely problematic and detrimental to patients seeking care for #Chronicpain and providers.

nature.com/articles/s4159…
This is not aligned with the role of nervous system sensitization, psychological factors, and other mechanisms underlying the chronic pain experience.
Imp to note that #machinelearning model from xrays explained only 16% of variability in pain overall

Just a friendly reminder @moderna_tx told the @SECGov that the @US_FDA defines #mRNA as gene therapy. And called it an unprecedented new category of #medicines tho they really want that #vaccine classification anyways for liability protections & fear the public wouldn’t take it.
They know the #COVID19 #vaccine isn’t a vaccine but human #gene #therapy.

In a recent lung cancer pilot study, we identified and quantified a record number of 13,187 protein groups with our next-generation single-shot proteomics technology HRM.
This was achieved using FAIMS-DIA with highest resolution chromatography and FAIMS Exploris 480. (1/n) Image
The depth of analysis was superior to the one achieved in the Deep Human Proteome Atlas (deep fractionated tissue with multiple MS acquisitions, Wang et al., 2019, Mol Syst Biol). Image
Comprehensively, we could identify 414 oncogenes, 392 transcription factors, 837 plasma membrane proteins (25 GPCR), and 316 kinases covering a dynamic range of 10. In addition, 0.25 Mio peptides (including modifications) were measured in totality.

I’m excited to launch @ABPI_UK's new report into the UK’s clinical research landscape (and my first as Head of Research Policy @ABPI_UK!) Here are my 6 takeaways 🧵...
1⃣ The UK has gone above and beyond when it comes to COVID-19 research with 6⃣8⃣ studies delivered in 2020 – the highest in Europe! 🔬🧪
2⃣When it comes to #GlobalBritain, only 🇺🇸 & 🇧🇷 started more C-19 trials in 2020. More broadly, the UK also punches above its weight in early phase research, maintaining its European lead in Phase I trials 🏆

Over the last year we’ve been voicing our charities’ stance on clinical research issues and helping to shape initiatives that influence the #clinicalresearch ecosystem.
🧵👇
When the pandemic hit, AMRC charities were forced to stop, pause, or delay 73% of the clinical trials and studies they fund to support the secondment of clinical research staff to the NHS frontline & the delivery of COVID-19 research.
Apr 2020: We worked with @NIHRresearch to develop a framework (bit.ly/2SPVwBd) to restart research. We also engaged with the Life Science Council’s Clinical Research Working Group & contributed to their report on transforming the UK clinical research environment.

Dr. Cook begins her #CCC50 Lifetime Achievement Award talk with reflection on her own 1st presentation ever as an #ICU fellow- at an @SCCM Congress! She was struck by the interprofessional group of attendees.(Thread)

#NationalWomenPhysiciansDay #WomenInMedicine #clinicalresearch
Her first lesson learned to impart to us.

BE BRAVE.

An early senior researcher said to her early in career:
"I just don't think research is in your future"

Imagine what #CriticalCare world would have lost if she had listened.

#CCC50 #pulmcc #PedsICU
Dr. Cook: Sometimes dialogue is more important than consensus. You have the TRUST YOUR GUT. #CCC50

You're not growing until you're changing. Defining moments for bravery may come along when you least expect them. Be brave. #WomensPhysicianDay #PedsICU #pulmcc #ICU

And then there is the question of who is white.

Iranian/Persian & Arabs are white on census.
Does the rest of America agree?

Racists & xenophobes tend to use loophole to say being anti-Muslim (whom they consider to all be Arab) is “not racist.”

latimes.com/projects/la-me…
Proving again: race is a social construct

Not until 2020 Persians really started to call selves “POC” (for self protection)

Have lots of Persian friends
Urdu culture has Farsi roots

But let’s be honest: Persians have seen selves as superior race, apart

Its Sunday and we’re doing a long thread on past medical abuses against Black communities to try to help science Twitter understand why “trust” is not the goal – the goal is empowerment. #blacklivesmatterinhealthtoo
When we talk about representation of Black folks in #clinicalresearch, the term “trust” is often used. “Trust” is too big a barrier and it can easily go away. The feeling is, “We’ve tried, but it’s too hard.”
But leaving communities out of advances in medicine is not the answer – we’re seeing the deadly results of this in #COVID19. But, still, we need to talk about why there is a lack of trust and why that is deserved. theatlantic.com/ideas/archive/…

1/ #ClinicalResearch Primer
A personal hands-on experince:
You’re interested in causes of ARDS & transfusion comes up. Literature review=many ?s: Is it the reason for transfusion or transfusion itself? Is it just fluid? Is it RBC or plasma. Is it “storage lesion” or antibodies?
2/ You dig deeper and find some fascinating studies largely known only by transfusion specialists:
ncbi.nlm.nih.gov/pubmed/4071603
ncbi.nlm.nih.gov/pubmed/11960539
You get your feet wet in a retrospective cohort study using a standardized definition (for the first time) ncbi.nlm.nih.gov/pubmed/16965572
3/ You learn it is plasma rather than red cells:
ncbi.nlm.nih.gov/pubmed/17400669
ncbi.nlm.nih.gov/pubmed/16276195
and the storage duration (fresh vs old) does not matter:
ncbi.nlm.nih.gov/pubmed/22281833
