Introducing the @DonateMask Project Canada! Need high-qualify #N95sForAll #masks? Request 'em at requestmask.ca - Got spare change (or more?) #donate a mask to a needy person at donatemask.ca
1/x
Biden's announcement today is the perfect kick to pull the cover off this project I've been working on: Since the Canadian Government and Provincial Governments have no reasonable plan to get high-quality masks to the most vulnerable Canadians, I saw a gap to be filled. 2/x
A bunch of my talented colleagues and I created the Donate A Mask Project Canada to address this need. The mission is simple: Putting high-quality masks in hands of the vulnerable with as little fuss as possible. 3/x

This is what targeted racial harassment looks like: nitpicking things like “smiled” or “wore clothes like a celebrity, as a celebrity” or “bought cookware.” It is part of the “gold digger” narrative.
This is common in #healthcare#professionalism” where there is hazing & bullying by racist &/or classist made up non-rules lacking standards…yet people are called “breaking the rules.”

It’s exactly how bullies operate: double standards, mislabel/smear, exclude, marginalize.
In the U.K., “BAME” includes the formerly colonized, thus making race constructs or power different from U.S. as latter had race-based chattel slavery.

Thing is, those formerly colonized have many who have internalized self hate & racism.

In July of 2021, the @CDCgov announced a decrease in US life expectancy of 1.5 years, the biggest one-year decline since #WWII. Even before COVID-19, however, the US has seen several years of declining life expectancy in the past decade. #ph260720 #COVID

Meanwhile, we see a widening gap in medical care between those with high and low status, leading to much larger declines in life expectancy for black and brown populations during #COVID19. How do you think these gaps came to be? #ph260720 #Healthcare
#hospitals continue to #Innovate and improve their medical care, following charters to improve health and care for the needy. However, they often fail to address the non-medical needs of their community, the “social determinants of health” #ph260720 #CommunityHealth

Will we EVER learn about #PublicHealth?
3rd wave “expert” committee is nearly-ALL #Pediatricians (to keep Devi Shetty’s non-#evidence-based assertion on #children will be entirely affected), #AllMale #Manel, nearly-all #clinicians headed by #PrivateSector #Cardiologist
I request this committee members:
- Please QUESTION Devi Shetty’s credentials to lead this
- Please hold to account #Manel #AllMale membership
- Please include #RuralHealth #CommunityHealth #PublicHealth
- Expertise is NOT only CLINICAL; also social; include grounded voices
Unfortunately no time & bandwidth; but putting this out there
We need:
- campaign for a DIVERSE expert committee including #Citizen #CommunityEngagement #Dalit #Adivasi voices
- Devi Shetty’s position as Chair is NOT tenable; appoint a #PublicHealth expert in his place

Doctors, especially female doctors, are rushing to defend Dr. Birx

“male-centered” world is problem = latest knee jerk “feminist” defense

Men cause female doctors to not speak on scientific facts in public?
Huh?

Why isn’t female doctors saving lives of WOC part of “feminism”?
The same doctors who will jump on the “racism!” bandwagon vs @JAMA_current if vs a *man*

will rush to cover up for a woman/peer who *operationalizes* structural racism

The face of structural & medical racism is female

And women will defend women rather than be held accountable
@DrJRMarcelin published this in @JAMA_current. Women of color, specifically black women, need to be leading the conversation on #COVID19 in America because when #publichealth systems fail on top of medical racism, you get exactly the results we got.

Trump was propped up by everyone standing besides him. Birx has blood on her hands.

“Leaders”, sadly, often sadly, women, like her are exactly why I left public service. Why would I rubber stamp death?

Birx and others like her are responsible for my mother’s #COVID19 death.
Compare Birx, who is on a reputation management tour, to a true #publichealth #servantleadership role model like Dr. Hasan Gokal

Is Birx defending him, her peer?

No, she’s playing victim exactly per the playbook of American-style feminism.

khou.com/article/news/h…
It was evangelical “science” that ruled the last administration - Birx included.

Where was Birx’s loyalty?

To vetted, verified facts and preserving human life?

I judge by actions and outcomes, not by “narrative” spun after a change of administration.

Dear #MedTwitter please, please, if you are a licensed medical professional and/or in school to be, do NOT delete your tweets or comply with anyone telling you to do so. It will harm you far more than leaving them up and explaining. Nothing is ever deleted. Can be used in court.
When I was at Harvard Med School and at an esteemed children's hospital, a case against an esteemed professor, that NO lawyer would touch cuz this was someone who had WRITTEN the textbooks and trained every expert, was won ..based on "a document was deleted" - creating suspicion
"Sunlight is the best disinfectant" you will hear me say OVER and OVER - wins & losses *should* be on data/standards but unfortunately wins & losses are in the court of public opinion. So be reliable, credible, and trustworthy. Transparency is one way to achieve that reputation.

Just because I am perpetually wading into controversy...as a BIPOC but not a Black woman... here I go again.

#BlackExcellence in #healthcare is VERY needed. Thanks for tweeting this @mdawriter

That includes #blackexcellence as physicians AND every other type of #HCW
This is super complicated. Traditionally, #healthcare has been physician-led "therefore" the ills of healthcare also fell squarely on the heads/shoulders (malpractice insurance) of physicians. That said, now most physicians are employed. Then the "therefore" of #accountability?
Workforce and representation very much are important for the 1:1 care. We know this to be true, for instance:

hbr.org/2018/08/resear…

But it is a long pipeline to get a practicing, licensed physician and requires a lot of debt/resources

This. Black nurses like @mclemoremr who not only have authentic clinical and community experience, not only know the disparities data, but also are winning grants to do the studies themselves belong on the transition committee or COVID Task Force.
In contrast when I have taught medical students straight from @AmerAcadPeds policy statements on racism in a hospital with a disparities center, have been told by a division chief that nurses find me “too political” which exactly is this that @COCoQC lays out

Not safe spaces Image
Repeatedly I “break ranks” - I don’t spend my time on outrage over “scope creep” - who is serving in the communities where Black women die at a 12x higher rate from maternal mortality? Not the doctors or nurses battling on own job & pay.

1⃣ Awareness. Most policymakers are challenged to understand how many #health workers they have, their locations, status & latest qualifications when making #health decisions. These challenges are exacerbated during crises like #COVID19 where real-time data are essential.
2⃣ Self-regulation. Effective human resource information systems are not built overnight. Most resulted through #healthpolicy mandates & required #multisectoral governance & buy-in, including health, IT, education, labor, finance, private sectors, as done in the #Philippines.

@sinanaral on false news

#tweetiatrician peeps, @AmerAcadPeds, @AmerMedicalAssn and @PublicHealth @DrTomFrieden @DrPaulOffit @SaadOmer3 @ASlavitt this is a must watch and Prof Aral’s work is so critical.

People>bots are source of misinformation spread

Must watch @TEDTalks by @sinanaral of @MIT & @MITSloan

Fake news spreads as it invokes surprise & disgust. It is also tied to novelty.

Being negative & relying on creating anger & disgust “wins” over positivity. *sigh*

@AmerAcadPeds #tweetiatrician

I’ve long had a concern over the doctors & #Scicomm who “review” “fake news” out there. Yes content may generate anger & disgust among pro-science followers but how often do you boost the stats of the original fake content?

Hear, hear

As a confident WOC willing to enter male dominated, non-diverse spaces (women >20% of my class & rare in tech)

I hear from men and women:
I take up too much space, I talk too much, etc, etc.

If invited 6x to podcasts in 2019, is that "too much"?
Who decides?
In the marketplace of ideas, lack of women o BIPOC in a space, what is the metric of too much/little? A recent thread on leadership coaching specific to #WIM: importance of having manners if one wants a seat at the table

How about results as the metric?

I'm successful as an advocate, having been a Health Advocate of the Year in 2012 by @Drsforamerica in 2012. Part of being an advocate is bringing issues to the forefront that may not be the priority of decision makers. One communicates on it (albeit, at times, relentlessly)

Let’s talk #safetyculture

@TheIHI

It’s a framework for a harm reduction towards all: patients & families as well as all staff

Important:
growth mindset
instead of
fixed mindset

to have needed uncomfortable conversations

#MedTwitter #AcademicTwitter #MedEd #NurseTwitter
This framework has been applied to reducing harm, mostly through medication error (though intended to achieve population health)

Then was expanded to patient experience

Also, cost of care

More recently we have added clinician wellbeing

=quadruple aim

ihi.org/communities/bl…
What has not been clearly stated in any of these #safety framework is disparities, structural inequity, or racism

The acute care side of the hospital - money maker - thinks of this as type of safety & #quality

but not #populationhealth

#Implicitbias elicits 🙄😡😴 response

Much debate on #SciComm. Little formal guidance exists. Those on academic committees that oversee ethics & professionalism are unfamiliar with social media. And now we are in a crisis and #pandemic of many levels. #publichealth very different from industry (1st vs rest).
In #publichealth one, the 1st points are be first & be right

There is an inherent tension between these two

With disrupted public health coordination, like cutting off from WHO, changing guidance from CDC (masks only for clinicians, no everyone wear them) how to be accurate?
In comparison, industry guidance #SoMe guidance. Industry has a lot more experience with this and uses it for marketing, market share, customer outreach, customer brand recognition, brand management. The focus is prestige, optics, reputation - there are monetizable traits.

A word to you ladies & femmes who are looking to advance equity:

Highly recommend studying cultures were women have historically held power, to understand use of power

I found this extremely informative, both on power & for my own cultural roots

m.startribune.com/daughters-kept…
A key attribute was that these women were daughters in tribal culture, therefore more trusted than an outsider

That can mean your university or company network, even religious group. For instance, Dr. Birx is evangelical as are current health advisors

khn.org/news/religious…
You still have to be qualified.
But final cut: usually “in group” gets in + staying power

“Group” can be defined in various ways. Options for me: #publichealth, #pediatrician, #communityhealth etc.

Know which groups have internal competition & dysfunction (Doctors, Academics)
