Did you know that pregnant people with #antiphospholipidsyndrome (APS) have an increased risk of adverse maternal & fetal outcomes? (1/4)
APS is characterized by #thrombosis and/or obstetric morbidity in the presence of #antiphospholipid antibodies. For pregnant people, this means increased likelihood of severe #preeclampsia, preterm birth, & #neonatalmortality. (2/4) 
Studies show that complement activity plays a key role in aPL-mediated #thrombosis & obstetric complications. And fortunately, complement inhibitors, like #eculizumab, offer promise of improved outcomes for pregnant people living with APS. (3/4)
32 M, previously healthy, admitted w/ acute ascites and portal vein thrombosis. Diagnosed with paroxysmal nocturnal hemoglobinuria. Eculizumab Rx advised. He is uptodate with all vaccinations. Apart from MCV4, what other preventive strategies should be considered?
(1/3) Ansr: MenB & PCN prophylaxis. Eculizumab: humanized monoclonal Ab against C5 complement protein ➡️ prevents formation of terminal membrane attack complex, a key factor in bacterial killing especially of Neisseria (meningitidis/gonorrhea). 10,000 risk of meningococcal infxn!
(2/3) Need to be vaccinated against N. meningitidis (quadrivalent MCV4 [Menactra, Menveo], & MenB [monovalent for serogoup B]) 2-4 wks before starting eculizumab. PCN prophylaxis should also be considered x 4 wks until last eculizumab dose (or longer for immuncompromised pts).
