Interestingly, you can see that most of the "asymptomatic" individuals were actually "pre-symptomatic" as many of us suspected

2/ It's unclear from this report if this is truly "reactivation." The authors mention that patients had symptoms, but it's unclear if these are new symptoms or persistent symptoms.

Is this really reactivation? Or just persistent infection? Or re-infection?
3/ It's all a little unclear, but it will be interesting to see more data

A positive PCR test just detects viral nucleic acid; it doesn't confirm actual live, replicating virus is still present. We don't know if these patients are still infectious (& they're likely not)

2/Not too surprising given that this is the biological role of Type I interferons in our body (i.e. our bodies' own antivirals).

Interferons are used for treatment in other diseases but definitely have significant side effects given their lack of specificity.

This study had no control group, so a randomized-controlled trial is still needed; however, these results contradict prior studies showing substantial benefits with this combination and should call into question the results of that study.

Clinical Metagenomic Sequencing for Diagnosis of Meningitis and Encephalitis | NEJM nejm.org/doi/full/10.10…
Of 204 cases, there were 13 cases where a pathogen was identified by metagenomic sequencing alone, and 7 of those results changed management.
Small yield for applying this technology to such a large sample of patients, especially since metagenomic sequencing alone missed 26 pathogens detected by routine tests.

As compared to other resistance mechanisms for Klebsiella pneumoniae, strains with carbapenamases were more likely to have their “genetic nearest neighbor” within the same hospital, suggesting they are more likely spread within single hospitals.
