1/🧵ICU Burn-Out Prevention in #COVID: a few thoughts
 
Early on as an ICU Doc, I carried a lot of shame because I realized that even though I sufficed as a technician and possessed the right knowledge, I was NOT doing a good job as a “person” with “people.”

2/ Which is to say, I was only “good” doctor on paper. NOT in real life.

I kept myself at too much of a distance, and I thought my job was to do procedures well and provide the right medicines.

Now I know that was not enough, not by a LONG SHOT.
3/ 📚 I wrote: “That shot of adrenaline from saving a life dissipated once the ventilator was in place…as if we handed over care of our patients to machines while we went off & intubated, sedated & paralyzed the next person...

A disabling malady from ongoing inflammation, autoimmunity, & potential viral reservoirs (GI, brain?)
 
NEW DATA: The Lungs “light up” on special MRI Scans 3 to 9 months later in patients never hospitalized for COVID.

2/ These #LongCOVID pts had NORMAL CT scans but when asked to inhale Xenon, an inert gas, the MRI showed their lungs transferred gases (think oxygen) into the bloodstream abnormally. This is alarming evidence of another weird form of organ damage from COVID not visible on CXR/CT.
3/ Oxford Prof Dr. Gleeson says these patients, who were never in hospital & didn’t have acute severe illness w their COVID19, have been experiencing symptoms for a year after contracting the infection.

1/🧵 People ask me, “What’s the main purpose of treating #COVID ICU pts with the #A2Fbundle? (a daily safety checklist of ⬇️ sedation & ⬆️ mobilization). It’s to prevent #PICS related dementia, PTSD, depression & reduce suffering in #LongCOVID.

HOW? What is the philosophy?
2/ For patients it’s about magnifying their #dignity and self-worth by liberating them sooner from us (our treatments that put them through a depersonalization chamber). For HCWs, it’s about purity of service, avoiding hypocrisy, and better self-sacrifice for our patients.
3/ Waking people up and walking them w family at their side in PPE (even inside a room if #COVID+) allows them to feel alive again. Repersonalized! Human. Seen. Drives away #delirium & #brainfog. Wouldn’t you want this?

1/ How do Children Suffer after ICU?

Today the mom of a 4 y/o boy who is in 📚#EveryDeepDrawnBreath wrote me & said to share this pic (w his twin). The family’s story can help us understand life after #CriticalIllness:

“Dr. Wes, we are 2 years out & he’s got a new limp!”
WHY?
2/ Mom writes: “Many appointments later we have learned that either his septic shock, ECMO, or the strokes caused damage to his growth plates. His left leg & foot are shorter than his right leg & right foot.”

This feature of pediatric #PICS will also affect sick #COVID19 kids.
3/ She goes on… “The doc at the children's hospital who we see said any of these 3 conditions can cause this to happen and our son had all three (strokes, septic shock, ECMO).”
