This is what targeted racial harassment looks like: nitpicking things like “smiled” or “wore clothes like a celebrity, as a celebrity” or “bought cookware.” It is part of the “gold digger” narrative.
This is common in #healthcare#professionalism” where there is hazing & bullying by racist &/or classist made up non-rules lacking standards…yet people are called “breaking the rules.”

It’s exactly how bullies operate: double standards, mislabel/smear, exclude, marginalize.
In the U.K., “BAME” includes the formerly colonized, thus making race constructs or power different from U.S. as latter had race-based chattel slavery.

Thing is, those formerly colonized have many who have internalized self hate & racism.

This 🧵 resonated. Trained in Houston, #asthma central, I was in this scenario SO many times.

I had the best #pediatric training at @TexasChildrens & @bcmhouston - you see it all.

Amazing that mere tweets can transmit humane, quality #MedEd teaching across the globe.
After moving to Boston I found myself surrounded by highly anxious doctors. We have an over abundance of resources and a healthier population. I saw people quick to overreact and manage (poorly) their own anxiety & need for control, with harmful aggressive overintervention.
I also do get being “Harvard” makes one a target. Juries are not going to understand “at the brink of death” and not intubating or the equivalent action in another scenario. “Jury of peers” is not present in a malpractice case. Good care can seem negligent, paradoxically.

We have lost >500,000 Americans to #COVID19 alone. Preventable deaths. These are mostly the marginalized, vulnerable.

Either document measurable work to save lives, or get out of the way and out of budgets that could be used to save lives.
I’ve seen SO much over the years, in the name of #compliance consulting to siphon off money to block &, ultimately, harm.

No this does not save money.
It harms families.
It monetizes coded racism.

better internal judgment & self management = protection
Is it any wonder physicians are unable to navigate increasingly complex healthcare policy or industry pressures if some unable to manage own selves?

“Physician led” slogan requires those issuing that rallying cry to demonstrate trustworthiness - else *you* harm the profession
#globalhealth, #Ruralhealth in America and #FQHC care require #teambasedcare, as does care for children with disabilities - so I am never going to mandate only one type of clinician or be locked into separate cult-like group coercion or shaming.

“male-centered” world is problem = latest knee jerk “feminist” defense

Men cause female doctors to not speak on scientific facts in public?

Why isn’t female doctors saving lives of WOC part of “feminism”?
The same doctors who will jump on the “racism!” bandwagon vs @JAMA_current if vs a *man*

will rush to cover up for a woman/peer who *operationalizes* structural racism

The face of structural & medical racism is female

And women will defend women rather than be held accountable

My mom? As a new immigrant to America, when she encountered an unfair Eurocentric rule from her English Lit grad program? She wrote a paper & got the rule changed

My mom and I are the same in core values but opposite externally. She uses maybe 1/100th of the words that I do and is intensely private. I am out climbing literal mountains while she prefers to stay in and read books and my dad’s #engineering journals.

My mom is an introvert who thinks 10x before speaking w/ surgical precision, educated by German nuns & British educational system

I am ALL for 1st amendment. Am anti-policing. Am anti-censorship.

Those of us in #SciComm can hold our OWN selves to higher standards than I see on #MedTwitter now. Vet. Verify. Be factual. Don’t do “left” vs “right.”
We cannot control what others post or do with our own posts to mislabel or misuse, unfortunately. We can reduce the “gotcha” aspects w/ passing things thru several gates. For me, having worked in the ED, been a public official, served as an expert witness, have automatic filters.
Deleting is NO protection. There is legal precedent “cleaning up” social media can lead to more questions.

It has been screenshotted. The “blocked!” or “deleted!” is used for innuendo.

This is super complicated. Traditionally, #healthcare has been physician-led "therefore" the ills of healthcare also fell squarely on the heads/shoulders (malpractice insurance) of physicians. That said, now most physicians are employed. Then the "therefore" of #accountability?
Workforce and representation very much are important for the 1:1 care. We know this to be true, for instance:

But it is a long pipeline to get a practicing, licensed physician and requires a lot of debt/resources

This. Black nurses like @mclemoremr who not only have authentic clinical and community experience, not only know the disparities data, but also are winning grants to do the studies themselves belong on the transition committee or COVID Task Force.
In contrast when I have taught medical students straight from @AmerAcadPeds policy statements on racism in a hospital with a disparities center, have been told by a division chief that nurses find me “too political” which exactly is this that @COCoQC lays out

Much debate on #SciComm. Little formal guidance exists. Those on academic committees that oversee ethics & professionalism are unfamiliar with social media. And now we are in a crisis and #pandemic of many levels. #publichealth very different from industry (1st vs rest).
In #publichealth one, the 1st points are be first & be right

There is an inherent tension between these two

With disrupted public health coordination, like cutting off from WHO, changing guidance from CDC (masks only for clinicians, no everyone wear them) how to be accurate?
In comparison, industry guidance #SoMe guidance. Industry has a lot more experience with this and uses it for marketing, market share, customer outreach, customer brand recognition, brand management. The focus is prestige, optics, reputation - there are monetizable traits.
