It took my legs a few minutes to warm up this morning, but glad to report that long morning walks remain easy, in contrast to later in the day. #Totnes is waking up beautifully ☀️
5 miles walked this morning, with no adverse effects. Then strimmed & mowed a lawn. Great to not be worried about my heart and lungs as I got a real workout from the gardening. However, I'm mindful of the warnings of some with #LongCovid that I should continue to pace myself.

Hypoxia --> low tissue oxygenation wrt demand

Types of hypoxia

1. Hypoxemic
2. Stagnant
3. Histotoxic
4. Anemic
5. O2 affinity hypoxia

2) The problem arises if you define O2 content of blood purely as PaO2 or as PaO2+ oxyHb saturation --> for this is the total O2 content of blood.

If we eliminate oxyHb, O2 content of blood becomes less meaningful since the lion's share of O2 is carried by hemoglobin!
3) If you consider oxyHb + PaO2 in total the anemic and O2 affinity forms of hypoxia must necessarily be a cause of hypoxemic hypoxia --> since anemia (reduced Hb) or increased O2 affinity --> cause HYPOXEMIA!

I want to talk about #LongCovid & #hypoxia & how my experience of it now is *drastically*, even *dangerously* different to what prior lung issues taught me.

I didn't plan to do this thread and I'm not thinking clearly atm, but it's important, so I'm going to give it a go.
Background: I was born prematurely & before synthetic surfactant, so I have asthma and temperamental lungs in general. I also have an extensive history of pneumonias that try to kill me (as in ICU stays). So I've had 40 years of knowing how to handle not being able to breathe.
I knew about happy hypoxia. I knew how, when, and what to do about it when my judgment about whether I was alright started to slip, and I knew that that's often the first thing to go when you're hypoxic - *first*, you feel... off. Unquantifiably off. It's easy to miss it.

I'd like to point out that while the resolution of a #chronicillness was striking, what is more striking is the rapid descent into Wernicke's in a hospitalized 90 yo woman - a descent so striking that she went into respiratory distress necessitating a #ventilator. 1/
Upon giving #IVthiamine per a #wernickesprotocol at 200mg 3X p/d she woke up, after just 2 days of thiamine treatment, and the ventilator was removed. What could this tell us about the use of thiamine in #COVID19? Perhaps the cluster symptoms we see are really just a variation 2/

Therefore Emergency doctors decide to admit otherwise healthy patients with oxygen saturation below 94% (young patient) or 90-94% (older patient or with chronic lung conditions). For those not falling below the threshold they perform the 40 steps test
