1/ I was asked by a friend what the current state of #COVID19 is, in practical terms. So I'll add to this thread slowly over time.
First off, every country will get 3 or 4 waves a year. It never was, and won't become any time soon, a seasonal virus.

2/ People still dying from #COVID19, everywhere at least ten times the usual death-toll from the flu. As-of-the-moment average death-toll from covid is estimated at 140 to 190 a day for the USA; keep in mind that's an under-estimate (b/c under-reporting) cdc.gov/coronavirus/20… twitter.com/i/web/status/1…
3/ Despite covid deaths being at very least 10x what's normal for flu in comparison (way over 10x for the USA just from flu alone), hospitalizations aren't so big at most times. However, one major worry is #LongCovid, in up to 10% (!) of covid-infections.

1) Eine "Immunschuld" (#immunitydebt) gibt es in der Immunologie oder der Public Health nicht. Das Immunsystem ist täglich mit vielen Erregern in Kontakt, aus dem Essen, der Luft und von
Oberflächen. Die Infektionsschutzmaßnahmen haben es also nicht "geschwächt" oder sonstwie geschädigt.

2) Die derzeitigen starken Wellen an ILIs haben mehrere Ursachen:
Speziell Bei Kindern: Hier sind durch die Infektionsschutzmaßnahmen der letzten Jahre die Kinder teilweise von
Erstinfektionen verschont worden. Diese werden jetzt nachgeholt (Kohorteneffekt). Daneben kann eine Schwächung des Immunsystems DURCH eine durchgemachte Coronainfektion eine Rolle spielen, das wird derzeit diskutiert.

On #immunitydebt
1- public health measures have limited exposure to diseases for a century. Yet, the phrase immunity debt just recently popped. When public health measures stopped exposure to waterborne diseases, people just lived longer, no faulty claims of immunity debt.
2- The age group most likely to be hospitalized by RSV are 0-6 mths. This age group was born during the "live with COVID" or "return to normal" phase of the pandemic. They didn't experience public health mitigations, therefore no "immunity debt" from it. cdc.gov/rsv/research/r…
3- and most importantly, your immune system is not a muscle, you dont need to continually exercise it to make sure it works. So public health measures that let us enjoy 2+ years w/out flu and RSV didn't cause your immune system to forget how to work.

This is a very succinct way to describe why I think the "immunity debt" excuse for our overflowing children's hospitals is...inane. But there are significant other flaws in the #ImmunityDebt theory, and alt hypotheses grounded in logic & data that make more sense. A 🧵 1/
1. Immunity debt assumes kids have been out of communal activities for the past 2.5 years. That's not true. Most ppl didn't have the luxury. We were extremely careful & extremely lucky to have been able to keep our kids out of congregate care, despite 2 working parents 2/
Grandpa *retired* to watch the kids all day. We took them out of school and therapies when I knew waves were coming (after Christmas, e.g.). At this point, MM has been back at school for 2 years. LO back in therapy FT for 6 mos. Most people didn't have that flexibility. 3/
