Female physicians have higher infertility rates and pregnancy complications

1. Female physicians, particularly surgeons, have a significantly greater incidence of #miscarriage, #infertility, and #pregnancy complications compared to the general population
doi.org/10.1097/sla.00…
2. In another study, of 692 female surgeons, 290 (42.0%) had a pregnancy loss, more than twice the rate of the general population.
*Female surgeons operating 12 or more hours per week during the last trimester of pregnancy were at higher risk of major pregnancy complications
3. Female surgeons had fewer children and were more likely to delay having children because of surgical training.
Incidence of postpartum depression and musculoskeletal problems were also higher in them

Short🧵Why I think #Covid can cause existential consequences for Human Race ?? For any species to continue it has to procreate, that depends on fertility of both men & women. We started getting edgy couple of months ago. Here's the original ctd
Ctd2) Then we found more correlation, the infection was really causing changes in hormone levels ctd)
Ctd3) We know that the number of ova in a woman is fixed at birth. The body doesn't produce more ova.Only a fixed number mature rest die off , maturation needs hormones & lots of other factors. If either hormones or reservoir is reduced #risk ctd)

Lots of discussion about lack of advancement in women's health

Here is the thing. We will NOT advance reproductive/pregnancy/women's health science by continuing to do clinical trials that are NOT based on fundamental mechanisms that drive health & disease

hang on...🧵🧵🧵
1/
Let me explain:

Millions (millions)💲 have been spent to perform clinical trials to decrease adverse pregnancy outcomes such as #preeclampsia

Trials on Vitamin C, E, magnesium, aspirin, calcium

Perhaps, you might be thinking--those are pretty generic interventions 👀

2/
And you would be right. They are.

Vit C & E clinical trial was based on concept that there is evidence of oxidative stress in placentas from preeclamptic individuals

OK. Yes, but oxidative stress is a pretty ubiquitous biological event

In honor of #EndometriosisAwarenessMonth: This is for the #MedTwitter community - especially #OBGYNTwitter - with the hope that it will help at least 1 future endo patient have a better experience than I did. A long 🧵: 1/?
3 weeks ago, I had an 8+ hours-long endo excision surgery that resulted in a unilateral #salpingectomy, an #appendectomy, & a partial #BowelResection. The thing is, it didn’t have to be this way. 2/?
I’ve had horribly painful periods since I was a teenager. Doctor said it was normal, just start taking ibuprofen a couple days early. By 18yo, oral contraceptives were recommended to manage it. That ‘worked’ for about 10 years? 3/?

We don't think this is clickbait, but actually an accurate and timley article which looks at the maternity scandal at Shrewsbury and Telford NHS trust, international commercial #surrogacy in #Ukraine and Catherine Bennett also highlights proposed #lawreform #surrogacyreform in UK
"The Law Commission’s confidence in British arrangements appears largely based on one small study of local altruistic surrogacy likely to be unrepresentative of any new, paying version." This possibly refers to Dr Kirsty Horsey's 2015 study...
Horsey, K., ‘Surrogacy in the UK: Myth busting and reform’ Report of the Surrogacy UK Working Group on Surrogacy Law Reform (Surrogacy UK, November 2015) "29. (27.1%) of these received less than £10,000, while 73 (68.2%) received £10-15,000 and five (4.7%) received £15-20,000."

Here is a prime example of how the baby-above-all-else narrative triumphs over fact in the #media: a 🧵. Let's start with criminal offences. In fact, overseas commercial #surrogacy is ILLEGAL for residents of NSW. Sydney, obviously, is NSW's capital. @smh fails to mention this.
International #commercialsurrogacy is also illegal for residents of the ACT and QLD. This article does not state this either. It fails to mention where these people are from. It does not question the legality of their actions.
Why is commercial #surrogacy a criminal offence in NSW? Well, because of 'considerable medical, ethical, legal and human rights concerns', which include the exploitation of surrogates and the resulting children. #fertility #infertility #law #ivf #donorconception #humanrights

New findings on #CovidVaccination and #Fertility!

Using the @BUPRESTOStudy, we show no association between #CovidVaccination in either partner and fertility among couples #TryingToConceive

A 🧵 on our new study in @AmJEpi #EpiTwitter (1/12)
doi.org/10.1093/aje/kw…
We enrolled couples who were #TryingToConceive without fertility treatment. Female and male partners completed questionnaires on lifestyle, health, and #covid19 infection and vaccination. We followed couples for up to 12 months to determine whether and when they conceived (2/12)
Then, we measured whether #TimeToPregnancy differed by vaccination status or infection history. We accounted for many factors (like occupation, income, etc.) that may be related to fertility and vaccination or infection (3/12)

"If you're on thyroid medication, Dr. Duke stresses the importance of checking in with your thyroid throughout pregnancy. "The fetal brain [and] spinal cord start forming at around 21 days, and the pregnancy hormone can actually activate your thyroid,” she explains.
“So, sometimes medicine needs to be tweaked in the first trimester. I check it with the first pregnancy test, and then once per trimester, and at delivery."

Check out this blog from @modernfertility as I answer the most common questions about thyroid problems.

Last year, our sister biotech company @LUCABiologics announced VIRGO, the first comprehensive reference in the gene catalog of the human vaginal #microbiome spearheaded by Chief Scientist Dr. Jacques Ravel. A thread 🧵
This database of nearly 1 million non-redundant genes reveals significant genomic and functional diversity within the vaginal #ecosystem, challenging the widely-held notion that an optimal vaginal microbiome is dominated by one strain. 🧵
This new view of intraspecies diversity has deep implications on the development of gene-centric, microbial therapies to target #diseases like bacterial vaginosis, urinary tract infection, and STI, which impact millions of women globally.🧵

That means you get it mainly from having unprotected vaginal, anal or oral #sex!

#Poly_Cystic_Ovary_Syndrome PCOS

PCOS is a HORMONAL DISORDER that affects Women of REPRODUCTIVE AGE (16 - 45 years of age).

1 out of 10 (10%) of ladies are diagnosed with PCOS. 🚺

#PCOS affects woman’s hormonal levels thereby AFFECTING the OVARIES.

A Thread 👇 Image
In HEALTHY WOMEN, the ovaries produce a very small amount of MALE HORMONES called as ANDROGEN.

But In PCOS, due to #INSULIN_RESISTANCE, the ovaries produce EXCESSIVE androgens, RATHER than the female hormones (such as Estrogen & Progesterone). ❌

These Excess #ANDROGENS are
responsible for CAUSING lots of body changes SUCH as : Facial Hair (beards) & No or IRREGULAR menstrual flow.

POLY-CYSTIC means ‘Many Cysts.’

In PCoS, Many CYSTS (Small Fluid-filled Sacs containing IMMATURE EGGS) develops inside the Ovaries.

The EGGS are NEVER fully grown to

Today is the start of #SHW2020! Each day this week, I'll be tweeting about Britain's long, tempestuous and ever-so-slightly bonkers love–hate relationship with its #sexualhealth.

Darlings, brace yourselves. 💋 #SHW20 Image
We begin with the Victorians, who, contrary to popular belief, were obsessed with sex. In the C19, the only known #STIs were syphilis and gonorrhoea. Diagnosis depended on observable symptoms (no blood tests/smears). Effective treatments were non-existent. Image: @ExploreWellcome Image
In 1864 the British government introduced the Contagious Diseases Acts, intended to protect the armed forces through 'regulation' of 'common prostitutes'. The Acts gave police almost-unlimited discretionary powers to arrest any woman they thought might be soliciting. Image: HCPP Image
