1) Circulating anti-GBM antibodies are directly pathogenic in anti-GBM disease.
However, about 10% of patients with anti-GBM disease do not have circulating anti-

Really pleased to share the results of the #LongCovid study I was part of with the Perspectum team out of Oxford University. Using a specialist new technology this focused on assessing multiple organ damage in 'first wave' patients, infected with #COVID in early 2020 or even 2019
We were given in-depth haemotological tests, respiratory assessments and most importantly whole organ scans using the new 'MRI' type enhanced scanning equipment. My first test showed #myocarditis, three liver #lesions, reduced #kidney function and multiple organ inflammation
Also in the first year I experienced #TIA, #atrialfibrillation, #septic bursitis, pre-diabetic symptoms, complete loss of taste & smell; hair loss; #polyarthritis joints, abdominal and bone pain plus multiple #infections and skin eruptions Some neuralgia, headaches and brain fog

The darker side of dark chocolates

1. Dark chocolates are popular due to their potential health benefits (they are rich in anti-oxidants and are good for heart) and moreover, they are low in sugar content. >50% consider them as safer and healthier option (as compared to candies)
2. But there’s a dark side to this “healthier” #chocolate. Research has found that some dark chocolate bars contain cadmium and lead—two heavy metals linked to a host of health problems.
Consumer report scientists detected cadmium & lead in many dark chocolate samples tested.
3. Consistent, long-term exposure to even small amounts of heavy metals can lead to a variety of #Health problems. The danger is greatest for #pregnantwomen and young #Children because the metals can cause developmental problems, affect #brain development, and lead to lower IQ.

Are "diet" soft drinks healthy?

1. People are opting for diet soft drinks in place of regular soft drinks, assuming that they are free of sugar, contain zero calories and hence are healthier & safer. But research shows evidence to the contrary.
#MedTwitter #HealthyFood
2a. In two studies conducted in US & Europe, individuals who consumed diet soft drinks had higher prevalence of metabolic syndrome than non-consumers.
Diet soft drink intake positively correlated with waist circumference and fasting plasma glucose in both studies.

A new milestone 🎉: my 3⃣0⃣th publication!

Published @amjtransplant, entitled:
"An interleukin 6-based genetic risk score strengthened with interleukin 10 polymorphisms associated with long-term #kidney allograft outcomes."
➡️pubmed.ncbi.nlm.nih.gov/36453708/

Time for a #tweetorial 🧵👇 Image
1.) Of all #kidney transplants, half are still lost in the first decade after #transplantation.

Using #genetics 🧬, we probed if interleukin 6 (IL-6) could be a target in kidney #transplantation to improve graft survival & if a genetic risk score could improve prognostication.
2.) Together with @marigaya, @VojtechPetr and colleagues from the @GTCumcg & @researchumcg, we studied the impact of IL6, IL6R, IL10, IL10RA & IL10RB 🧬variants on late graft loss by analyzing both recipient and donor DNA of >1,200 #kidney transplant‐pairs.

Empagliflozin is now my go-to SGLT2i for pts with #diabetes and #CKD or #CHF, over cana or dapa or ertu.

Why?

A: EMPA-KIDNEY: nejm.org/doi/full/10.10…
and EMPEROR-Reduced and EMPEROR-Preserved
1/
EMPA-KIDNEY is unique because it enrolled GFR 20-45 *without* proteinuria whereas others (DAPA CKD, CANVAS) enrolled pts with CKD+proteinuria, though DAPA-CKD conducted a subgroup analysis for UACR < and > 1000
The primary outcome was a composite of progression of #kidney disease (defined as #ESRD, a sustained decrease in eGFR to <10, a sustained decrease in eGFR of ≥40% from baseline, or death
from #renal causes) or death from #cardiovascular causes

Thread on #Magnesium and why it is fundamental for our #health. #micronutrients
..many people debate the various ratios of #macronutrients like #protein, #fat, and #carbohydrates in their diet to find their optimal health. Which is great as we are #bioindividuals but most miss out on #micronutrients like #vitamins and #minerals.
The way our bodies are able to use the #macronutrients is fully dependent on how our #enzymatic pathways are influenced by various #micronutrients. One of the most important amongst is #magnesium.

@Mktweets0 Going to start a thread on #melatonin & its effects on CKD validated by scientific research. Please correct if you find any medical faults.

#kidneys
@Mktweets0 1/ Melatonin was first purified and characterised from the bovine pineal gland extract by Aron Lerner and co-workers in 1958.

However it has systemic effects. #Kidney cells (proximal tubule) express a melatonin receptor of type MTNR1. pubmed.ncbi.nlm.nih.gov/9034171/
@Mktweets0 2/ Looking at mRNA expression across several human tissues, surprisingly it's rather strongly expressed in colon and #kidney.

Study from @salkinstitute found that partial reprogramming using the #Yamanaka transcription factors (#Oct4, #Sox2, #Klf4 & #cMyc) for short periods of time:

-restored a youthful #epigenetic signature to #aging cells & extends the life span of a #premature #aging mouse model.
-long-term partial reprogramming led to rejuvenating effects in different tissues, such as the #kidney & #skin, and at the #organismal level; duration of the treatment determined the extent of the beneficial effects.
-The rejuvenating effects were associated with a reversion of the #epigenetic clock and metabolic and #transcriptomic changes, including reduced expression of genes involved in the #inflammation, #senescence & #stressresponse pathways.

My sincerest & humblest apologies all! For the first time in my life, ever, 9pm, Christmas Eve & i actually just realised what day it was! 😱⚠️
No, i have no idea either but i guess that's what 2 years of near isolation can do to you (throw in painkillers, CF, fibro & depression)
Never assume that because you don't hear from someone everything is ok (the old, "no news is good news" {as all must be ok!}, they're just busy! Especially when it's such a landmark day & ppl are invariably wrapped up with their families. That's not always the case!
Why am i isolating at Christmas? 2 reasons:
🔵 many NHS staff appear to be falling ill despite being triple jabbed &
🔵 Studies seem to be showing effectiveness behind to wane a bit after 10 weeks.

So do you go meet your elderly mum, who's allowing my nephew to shuttle between..

This paper has many interesting findings but I wanted to focus on the #brain-#kidney axis. What is the relationship between the kidneys and brain injury in malaria? Can measuring tau levels help us identify children at risk of long-term brain injury? Let's discuss! 🧵 Image
Last year there were an estimated 627 000 malaria deaths with 80% of African deaths occurring in children <5 years of age. Unfortunately, due to COVID-19 disruptions in public health measures to control malaria, this is higher than in previous years.
Survivors are at risk of long-term brain injury. We need tools to identify children at risk of brain injury for early interventions so that every child has an opportunity to survive, and thrive.

... more from the #ESCCongress2021 from the @GoggleDocs #Takeover of the @cardiomet_CE

Next up the EMPEROR-Pooled analysis and some of the questions it asks

How does empagliflozin compare with ARNI (sacubitral/valsartan
2)
EMPEROR-Pooled was a prespecified analysis of the two 'sister' studies

EMPEROR-Reduced Trial
nejm.org/doi/full/10.10…

and

EMPEROR-Preserved Trial
nejm.org/doi/full/10.10…

With very similar #HeartFailure populations, separated by an ejection fraction cut off of 40%
3)

As you can see

📍Very similar results in terms of the primary and secondary #HeartFailure outcomes.

‼️ Although clear attenuation of benefit seen in pople with higher ejection fractions (EF)
