Major update for small cell #lung cancer! #LCSM

With @juliensage + @FoundationATCG, our paper now on @bioRxiv:

►Genomic analysis of *3,600* pts with SCLC; largest SCLC study to date!

►Multiple interesting findings discussed in this 🧵
biorxiv.org/content/10.110…
🧵2
This study is unique in many ways:

> The #SCLC cohort studied came mostly from community sites --> representing a more typical “real-world” cohort of SCLC.
> The genomic data are tied to clinical data + outcomes.

#LCSM
🧵3
To the best of our knowledge, our study analyzes the largest number of SCLC tumors from African ancestry to date:
> n=256 AFR cases
> 7.1% of the entire cohort
#Diversity in clinical studies is critical!!
See tweet 🧵9 for more details.

1/As promised & after reading $NTLA latest Q1 2022 financial report here is my analysis regarding @intelliatx latest corporate status. I have focused only on the main issues that I found to be the most interesting & relevant. #CRISPR #GeneEditing #BioTech #FinTwit #Genomics👇
2/NTLA-2001 is the most advanced In-Vivo #CRISPR/#Cas9 #GeneEditing program in $NTLA portfolio & is aimed to treat Transthyretin Amyloidosis - (ATTR). NTLA-2001 could potentially halt & reverse the disease in a “one time” treatment by knocking out the TTR gene with a single dose
3/In February $NTLA has presented an updated clinical data from its Phase 1 study of NTLA-2001 which included 15 patients with ATTR amyloidosis. IMO NTLA-2001 readout looks promising & here is a summary that I wrote shortly after @intelliatx’s presentation of NTLA-2001👇

#location location location: thrilled to share our work @CellCellPress! #interferons dictate #COVID19 severity! Specific #IFNs are produced along the #airways in response to #SARSCoV2 and drive protective or detrimental #transcriptional programs! 1/7 doi.org/10.1016/j.cell…
IFNs are driven by high #viral #loads, suggesting that viral recognition is only achieved when #SARSCoV2 reaches a certain threshold! Efficient induction of protective IFNs is preferentially achieved by younger patients, further explaining susceptibility of elders to #COVID19 2/7
#IFNL1 & #IFNL3 characterize mild illness & drive potent anti-#SARSCoV2 #ISGs in the upper #airways. Severe patients show #typeI IFN & #IFNL2 along the respiratory tract but lack ISGs, dampen cell proliferation programs associated to #repair and boost #apoptotic #p53 pathways 3/7

1/ This excellent study by @aaronmring/@VirusesImmunity and team found that COVID-19 patients exhibit dramatic increases in the production of antibodies against thousands of human extracellular and secreted proteins (the exoproteome) compared to controls 👇
2/ The million dollar question is: what molecular mechanisms underly this antibody/#autoantibody production? It is worth interpreting the findings via the lens of human #microbiome/#virome activity + the activity of persistent pathogens (such as EBV) harbored by study subjects.
3/ Every study subject harbored extensive microbiome/virome communities comprised of trillions of organisms during #COVID-19 infection…with such ecosystems now understood to persist beyond just the gut but also in other body sites (#lung, liver etc).

#Thread #COVIDSecondWave
Dire warning from Dr Darayus Gazder, a young doctor in #Karachi: Second wave is very strong, patients are highly critical and sick. We have a waiting list of over 10 per bed. People are dying/suffering alone w no one to say goodbye!
It's painful
1/n
Some deteriorate within minutes. Others who survive COVID have permanent severe #lung damage, can't eat a single meal w/o high flow oxygen, later die painfully as if #drowning once respiratory/cardiac muscles can't compensate as oxygen is depleted.
- Dr Darayus Gazher 2/n
Educated people should take this seriously. But they are Instead throwing parties, weddings, going to sports events, malls, shopping, eating out with no precautions, even mocking the disease. This selfish behaviour must end as it endangers ALL age groups

How long do #neutrophils live? Are they all equal or do they adapt to tissues?
Get answers to these & more questions in our new paper just out online at @CellCellPress!
Led by our fabulous @AndreaRP88 & @ivanbamartin!
Key points in thread 👇

1/4 Saw this at Bandstand today. As a chest physician, this made me very happy! Pigeons are responsible for an interstitial lung disease (ILD) called hypersensitivity pneumonitis or extrinsic allergic alveolitis. Image
2/4 Most chest physicians and radiologists in the city will tell you how the number of patients with the disease has consistently increased over the years, paralleling the increase in the pigeon population of the city.

Two #ASGCT20 morning sessions focused on lung delivery. Cool work, but it seems to me that the big issues for clinical stage haven't really been addressed:
#AAV can target specific cell types in the lung, and multiple groups have rescued mouse models of genetic diseases. But both new and old lung-trophic capsids seem to hit T2 epithelial cells, which turn over and lose non-integrating transgenes.
Why does the virus end up in these cells? Are the T1 cells designed to reject microbes? What about fibroblasts and other cells, that do get transduced in some tissues?

#Remdesivir begins distribution in the #USA today via #EUA.

I thought would write down some practical #tips based on my #qualitative anecdotal experience after personally treating close to 200 patients with #COVID19.

For quantitative results, wait for the papers¬—thread
“this juice works, doc!” When you hear similar expressions 1-2 days after starting #remdesivir multiple times, you start to wonder. Don't recall a similar proportion of patients smile after feeling so sick 1-2 days earlier with #COVID19. x/n? Wait for the papers.
Like #treatment for any severe infectious disease (meningitis, Gram-negative bacteremia, influenza pneumonia, etc), earlier is always better. #COVID19 is no different. #Remdesivir stops #SARS-CoV2 replication, but won’t heal the lung injury.

THREAD from the new #bradykinin research (pre-print) from 🇳🇱:

"We propose that the severity of the disease and many deaths are due to a local vascular problem due to activation of B1 receptors on #endothelial cells in the lungs."

#COVID19 #kinins

preprints.org/manuscript/202…
➡️"the B1 receptor on endothelial cells is upregulated by proinflammatory #cytokines

➡️ Without #ACE2 acting as a guardian to inactivate the ligands of B1, the lung environment is prone for local vascular leakage leading to #angioedema"

...

#bradykinin #COVID19
➡️"... and might explain the typical #CTscans and the feeling of people that they drown.

➡️In some pt., this is followed by a clinical worsening of disease around day 9 due to the formation antibodies directed against the spike (S)-antigen of #SARSCoV2 that binds to #ACE2"

Let's journey back on the #FireTheSG movement, shall we?

December 2018, @Surgeon_General @JeromeAdamsMD convinced the public #media, inflating a false "epidemic" w/30-day use, not daily.

Lie #1: vapinglinks.blog/2019/01/06/epi…

Based on feelings, Google alerts, potential & inaccuracy.
No refuting vaping products help smokers quit, 👆above, continues below...

With #data he wanted to present, sentiment and "feelings" continued.

Lie #2: rodutobaccotruth.blogspot.com/2019/11/high-s…

Indifferent, bans & taxes ⏫increasing countrywide, & no care of helping smokers quit.

#FireTheSG
False statements, accusations continued. When corrected, he didn't like that and had to swallow his narrative.

In full, with @2Vaping, @TheLegionCmdr, @Surgeon_General & others:

A fisking of the opinion piece by Michelle Jongenelis (from the doghouse of @DrOliviaDoll1) in Fairfax media. No public link known (it appeared in a variety of regional newspapers around #Australia).
"THE recent death of a US man who contracted a serious lung disease after taking up #vaping comes as no surprise to many #PublicHealth advocates."
No mention of #BlackMarket, unregulated #cannabinoids, despite 6 US regions involved, explicitly mentioning it.
"Contrary to the messages being disseminated by the vaping industry (an industry now largely owned by #BigTobacco), .."
Wrong! Neither the majority of companies, nor vape sales, involve products ownership by the #TobaccoIndustry.

Hey #tipsfornewdocs #medtwitter folks! I wanted to review side effects of immune checkpoint inhibitors (ICIs) for a non-#oncology audience. Immune-related adverse events (irAEs) present across all medicine specialties so we are all in this together. 1/x
The most common ICIs are against PD-1 (e.g. nivolumab, pembrolizumab) and PD-L1 (e.g. atezolizumab, durvalumab). I use a blanket term “PD-1” throughout. Anti-CTLA-4 (ipilimumab) is a distant second; often in combo or directly after PD-1 in #melanoma #lung #kidney cancers. 2/x
They have led to pharma riches and Nobel Prizes and lots of TV commercials because they work really well for some patients. People with advanced #melanoma, #lcsm etc who used to live ~1 year now can hope to live for several years or more. 3/x
