Also, at midcareer female success is threatening to men our age AND to younger men who rely on patriarchal patronage systems. Women, overpoliced by the system, then, yes, are tougher bosses AND we *do* tend to adhere more closely to ethics.

I do do all:
On #MedTwitter there is a pattern of “callouts”, yes vs all genders & ages, but also pattern of misogyny and misogynoir. It happens towards nurses & doctors alike. One of the reasons I connected with @ShrimpBrokkoli is because I saw a yt male resident ridiculing her online.
I saw that behavior being taught by NON-clinical faculty & that REALLY crossed a line.
If you are a woman + nurse + frontline + Black/WOC, you already live with so much violence towards you. Added online made up drama from a trainee vs nurse?
No, no, & no.

This is what targeted racial harassment looks like: nitpicking things like “smiled” or “wore clothes like a celebrity, as a celebrity” or “bought cookware.” It is part of the “gold digger” narrative.
This is common in #healthcare “#professionalism” where there is hazing & bullying by racist &/or classist made up non-rules lacking standards…yet people are called “breaking the rules.”

It’s exactly how bullies operate: double standards, mislabel/smear, exclude, marginalize.
In the U.K., “BAME” includes the formerly colonized, thus making race constructs or power different from U.S. as latter had race-based chattel slavery.

Thing is, those formerly colonized have many who have internalized self hate & racism.

This makes me so 🤢

The idea that women are automatically “safe” is exactly what creates this “opportunity” for some women to exploit children & monetize their “optics” of “trustworthy” on behalf of predators.

Gender does not predict ethics, integrity, safety.
Why does #GhislaineMaxwellTrial matter to #womeninSTEM?
⬇️federal funding
➡️private donors (Epstein)

“Socialites” open doors to elite spaces, including #academia #science #STEM
Women=“optics” & “reputation management” for these wealthy predator men

theverge.com/2019/9/19/2087…
These same connected women, often daughters of powerful businessmen

are on boards of #WomeninSTEM “empowerment” organizations or on #MeToo (TimesUp scandal)

the very women who are predators of young women are given access to women’s “safe spaces”? (!!!)

#antivaxx efforts now even use #deepfake

to target #refugee, Black, religious minority communities

like measles outbreak in Minnesota in 2019: Wakefield was flown in TO spread #misinformation & #disinformation

Now: online fake accounts/fake identities vs #VaccinesWork
What has #healthcare been doing meanwhile?

#Medbikini: Teach (male Asian) med students to make fake accounts to cyberstalk trainees’ personal Facebook for: bikini
or “being political” or “religious” (#BLM)

➡️“model minority” pipeline for “shadow faculty” vs Blck @ayshakhoury
#Medbikini #research got flak for men vs women

BUT was ALSO fake acct #surveillance vs #BLM like drove out @ayshakhoury @uche_blackstock @nhannahjones

While academia has ignored #vaccineequity

The only feelings to center on are those of black community
Wrong use of this quote during the week of #DerekChauvinTrial

Some people “feel” things if a white male faces accountability
Amy Cooper’s feelings: call cops
Racism & racism lite has always focused on “feelings” #BLM
When @ayshakhoury or @uche_blackstock were authentic in their feelings: ended careers

Black female physician #DrSusanMoore having “knowledge” or a “voice”
while DYING
made others
*feel* “intimidated.”

This is part of a known pattern for WOC
@COCoQC
#BLM #BlackintheIvory
This is a repeating pattern of “feelings” used against tellers of uncomfortable truths that #MLK described in the letter from the Birmingham jail.

Seeking order > justice or overpolicing are all to protect only certain “feelings” of “security” while those like #adamtoledo die.

In light of the Boston police Union leader child abuse, this is very plausible, even probable. I have 2x, on behalf of someone else, reported, via Title IX - the woman or gay man was not feeling safe enuf to report. Yes, there is blowback when one reports. But we can’t be silent.
I am a mandated reporter as a #pediatrician. I *must* report for my patients or face legal consequences. Am same for staff, students, trainees. But in #EmergencyMedicine who do I call if a child may be unsafe at home? Sometimes, cops. Are cops safe?

boston.com/news/local-new…
I have seen #mentalhealth and entire life paths destroyed by abusers preying on children like: This happened to too many people I know. I have seen institutions intimidate, mislabel, bury stories of the victims. (Some predators wait until 18th birthday.)

I have a range of writing & #communication styles

Outside the ED of hospitalist medicine, when I “talk like a man” - direct, unvarnished truths and or “taking up too much space”, there is pushback, affront. Others have noted men with same style as me are👍
#WomeninSTEM
#SciComm
In reality women can’t just email or communicate “like a man” for same results. Society *expects* different communication from women.

When you want to know why
-women & minorities leave medicine
-why research lacks data to serve needs of anyone not white & male
and/or
-why different mortality rates in patients by gender, race, SES

this is why

This is #MedBikini “#professionalism” again

#MedTwitter
#MedEd
Is insidious
but highly effective
maintains
-segregation
-different access
-barriers

As women, LGBTQ, minorities, disabled enter or advance in medicine, this professor’s public stance will be cited to mislabel having #ethics as “lacking #professionalism”

bioethics.net/2020/07/medbik…
#medbikini is not one article
=attitude of many physicians
=drives this

We saw this happen with @ayshakhoury and @uche_blackstock and so many others. It especially happens at midcareer to WOC, especially black. Also if Muslim, LGBTQ, disabled, #mentalhealth

Cyberstalking by faculty and administration is simply never okay. #Medbikini was not a one off.

Almost everyone, regardless of age & gender, whose bio has “#professionalism” in it or “#cybersecurity” is worth looking into as to what #ethics they disregard or violate.
#Mentalhealth is needed. Until they clean up their #ethics minoritized populations will continue to be unsafe

Torture relies on “exploiting” psychological or physiologic vulnerabilities

Some, instead of creating #health, sell services to design torture

washingtonpost.com/outlook/psycho…
Conflicts of interest, especially unreported, are a massive source of ethical failure. First, clinicians should inherently know torture is unethical as well as “unprofessional”

but if they lack a moral compass or ethics, reporting COI can flag to others

If you want to understand a woman, look at her mother - at least true of me

My mom? As a new immigrant to America, when she encountered an unfair Eurocentric rule from her English Lit grad program? She wrote a paper & got the rule changed

Smart
Owned her power
Effective
Ethical
My mom and I are the same in core values but opposite externally. She uses maybe 1/100th of the words that I do and is intensely private. I am out climbing literal mountains while she prefers to stay in and read books and my dad’s #engineering journals.

My mom is an introvert who thinks 10x before speaking w/ surgical precision, educated by German nuns & British educational system
We are Urdu-speaking

She is fearless in face of patriarchy

It runs in the family. Have strong, activist #womeninleadership role models in #Pakistan

Agree and...

Many of us also saw an ugly side of ivory tower

multiple out of touch clinicians & non-clinical faculty overpolicing minority, LGBTQ, women clinicians, trainees, students

Revealing what drives distrust of institutions/faculty

Must fix this for effective #SciComm
#SciComm currently lacks Black or marginalized group (including BIPOC, disabled, etc) leadership within it which explains why so called leaders within it cannot recognize effective public health messaging that works in communities of color, marginalized groups.

Need more:
This video shared by @KateWalshCEO of @The_BMC represents a divide in #SciComm & #MedTwitter & #professionalism
Some find clinicians dancing offensive
The thing with diversity is not everyone at an institution is on the same page, evidenced by #Medbikini

The entire field of #professionalism is rotten to the core, racist, sexist, ableist

=glorified cyberstalking taught by creepy older men
carried out by younger folk
as we saw from #MedBikini

Academia rewards this w/ peer reviewed publications

bioethics.net/2020/07/medbik…
I first started to see it when I worked in #Medicaid on #compliance side, some women in particular took excessive pleasure & identity being “the policewoman.” At the time was starting to learning the preschool to prison pipeline (but NOT from @AmerAcadPeds)
= scary underpinning
Who polices “the police”?

There are SO many jobs that need to go away

that merely exist cuz someone whips up fear, hatred, disgust
then monetizes it

“Let me police ‘those people’ for you”

while delivering not zero value but NEGATIVE value

= Waste

Attacks range from on practicing clinicians by lay people to attacks on trainees by faculty. #Medbikini was faculty teaching trainees & med students how to stalk their peers, especially young women in the profession. Attacks can come from inside, often as “professionalism”
“One of our really big concerns is that physicians and scientists who are women, people of color, LGBTQ individuals and underrepresented minorities are the ones who are more likely to be attacked on social media”

Keep in mind attacks can be external or by faculty vs “lower”
Attacks from within profession are under the label of “professionalism” to mask it being stalking of trainees.

Look at the #Medbikini “professionalism study” and compare to what we know of professionalism being inherently racist.

Even if cyberstalkers had been women, not ok. ImageImageImageImage

It “took a village” to “raise”/create Andrew Wakefield - it is common for everyone to back away once accountability happens but Andrew Wakefield did not do this all by himself.

What about medical culture and who is not policed on professionalism that allowed him to get so far?
When I look at Wakefield, I see the same kind of lack of ethics evident in #Medbikini study

but Wakefield was far further down the spectrum

because he was doing invasive GI procedures on children for his study, funded by a lawyer

First: consent matters
Informed consent matters
What incentives exist in healthcare & academia that shaped his mindset & behaviors?

What was Wakefield rewarded for throughout his career?
Where did he NOT get questioned?

Look as this patent he had filed to compete vs MMR

Dear #MedTwitter please, please, if you are a licensed medical professional and/or in school to be, do NOT delete your tweets or comply with anyone telling you to do so. It will harm you far more than leaving them up and explaining. Nothing is ever deleted. Can be used in court.
When I was at Harvard Med School and at an esteemed children's hospital, a case against an esteemed professor, that NO lawyer would touch cuz this was someone who had WRITTEN the textbooks and trained every expert, was won ..based on "a document was deleted" - creating suspicion
"Sunlight is the best disinfectant" you will hear me say OVER and OVER - wins & losses *should* be on data/standards but unfortunately wins & losses are in the court of public opinion. So be reliable, credible, and trustworthy. Transparency is one way to achieve that reputation.

@BaeBielle Excellent questions!

These are the questions all of us should always be asking.

There are many best practices for doing stakeholder engagement to come up with a good multidisciplinary approach.

Some examples: ImageImageImageImage
@BaeBielle the current status of “professionalism”

is that a few people call selves “experts”

is a career lacking data or EBM

criteria are opaque, inconsistent, biased, decided by individuals

#Medbikini revealed lack of standards and professionalism failing on EBM standards
@BaeBielle I have had enuf roles, including in compliance & overseeing standards a “professional” behavior, has been my job, like for the whole state or for 700+ employees as VP of medical affairs

I see patterns of bullying & abuse of power

Or this vs @timnitGebru

Frankly every single person has some kind of privilege. If you have sight or two functioning legs you lack certain barriers

Acknowledging the same about structural racism is not insulting

unless you have cultural chauvinism/superiority & believe your own culture faultless
Personally, as someone Muslim, what I was taught about my purpose on this earth is the every single thing I “have” is only loaned to me and nothing I am entitled to.

You have to pay interest on loans.

More you have, more you must give (back).

Of course I face barriers too.
I don’t talk about religion a lot. I am deeply spiritual. I do believe Islam is “a way of life.”

I am less worried about reciting the exact right dua/prayer when stepping in a room with the correct foot.

Hadith can be disputed but this one is beautiful. Mercy matters most.

#medtwitter - please realize that deleting tweets & blocking

offers NO safety

In fact a very high profile case at a Harvard hospital was lost merely on “something was deleted”

it only raises suspicion about you. Leave it up. Do NOT delete/erase.
anyone teaching you documentation, communication, or social media who deletes, teaches you to delete

is sending you off a cliff of self destruction

I used to work for the state, oversee credentialing & discipline, and served as an expert witness for the AG’s office. I know.
What I was taught, back when we had paper charts, was to put a single line though, initial, and date. Then correct. When compliance types come through they will have MORE confidence in you, in fact. You had the good sense to self assess BEFORE they got there and adjust/improve.

@ayesha_mian1 @WEIconnect @UN_Women @ELAMProgram @AfafMeleis @_saadshafqat @WGHPakistan @DrvanTilburg @arghavan_salles @JulieSilverMD I would go further. A woman who has actually earned a degree is told her actual role in the world is defined by “wife.” A publication with mostly male editors, read mostly by men, in the finance sector publishes a poorly written piece ridiculing her & rewarded with “clicks”
@ayesha_mian1 @WEIconnect @UN_Women @ELAMProgram @AfafMeleis @_saadshafqat @WGHPakistan @DrvanTilburg @arghavan_salles @JulieSilverMD She is told she is unqualified or cannot use her earned credentials by a man who lacks credentials. As a physician, can say this about doctor culture in the U.S.: we are a socially stunted puerile bunch easily manipulated by sales & marketing ppl or by metrics of competition, obsessed with status, prestige, ego..

...not my doctor friends...but we hang out at the margins.
The high school level drama that plays out in hospitals, then the replica, amplified, on #MedTwitter is 😱 No wonder our profession kills so many patients when we are so busy policing on pettiness & ignoring standards of quality, safety, population health ncbi.nlm.nih.gov/pmc/articles/P…
There are many reasons why I am hanging up the stethoscope, one of which is that I cannot in good faith go into any clinical setting and feel confident about “first do no harm” - The system is too misaligned, not driving quality, safe results

Then, the human dynamics are

Accountability is when someone takes money for a job and fails to use standards or violates standards. Referencing standards when seeking accountability, in a transparent way, is fair game and necessary. Teaching at esteemed institutions = an obligation to deliver quality.
Some info on #MedEd - the roles and responsibilities

medschool.ucla.edu/workfiles/site…
For medical professionals, especially doctors, payment of the critical part on liability.

Good Samaritan protection relies on no payment, often.

Doctors can be served papers up to 25 years later if it was an infant.

That is level of accountability of clinicians

I know that there is the anti-leader leader concept (not using titles, wearing hoodies and such) particularly in the tech & entrepreneurship world. At the same time, those are spaces that are not necessarily as woman or BIPOC friendly historically so the "casual" can be a trap
implicit, or frankly, explicit biases have not gone away. Except professionalism is also a trap - it is well documented in peer-reviewed publications and I've repeatedly notice with how multiple "professionalism" faculty fail to examine embedded biases & cultural chauvinism
In tightly controlled spaces there is a so-called right/wrong way to have one's hair, with "wrong" including natural hair if one is "ethnic", women must wear skirts and pantyhose automatically adding distracting details (do I have a run in my stocking? am I sitting okay?) &such

@sinanaral on false news

#tweetiatrician peeps, @AmerAcadPeds, @AmerMedicalAssn and @PublicHealth @DrTomFrieden @DrPaulOffit @SaadOmer3 @ASlavitt this is a must watch and Prof Aral’s work is so critical.

People>bots are source of misinformation spread

Must watch @TEDTalks by @sinanaral of @MIT & @MITSloan

Fake news spreads as it invokes surprise & disgust. It is also tied to novelty.

Being negative & relying on creating anger & disgust “wins” over positivity. *sigh*

@AmerAcadPeds #tweetiatrician

I’ve long had a concern over the doctors & #Scicomm who “review” “fake news” out there. Yes content may generate anger & disgust among pro-science followers but how often do you boost the stats of the original fake content?

Have also seen sharing of false info about #STEM peers

Woah..that was freaky but familiar. Was lying down & decided to get up to take pain meds for #endometriosis and #fibroids and it felt as painful to get up as the day after surgery. In kitchen, sweating, lightheaded. Quickly squatted to avoid passing out holding onto fridge door
..which swung open..but I did not fall. Then once head feeling better, stood up long enough to take meds...then walked, wobbly and lightheaded to chair. I can't quite feel my palms but that will come back. This probably gets to be called 10/10 pain..but maybe really an 8/10
Great..and a migraine too..scotoma.. taking a migraine med and lying down. Man, when it rains it pours.

Mother Nature is such a misogynist.

SO glad I did not decide to rent a car and try to go Finger Lakes this weekend to see peak foliage.
