Here's a physiology case that *everyone* who touches a ventilator needs to understand:

A 60 yo woman is intubated for hypoxemia from multifocal pneumonia.
She has a SpO2 of 89% on PEEP +12 and 100% FiO2.
PEEP is increased to +16 & her SpO2 drops to 80%!

What happened?

1/
Before we get into the answer, let's make it interesting with some multiple choice.

Which mechanism(s) could cause worsening hypoxemia with increasing PEEP?

2/
Wow! 6000 votes! Im amazed by how many people share my love for šŸ« physiology!

Iā€™ll post the answer tomorrow. If you canā€™t wait thereā€™s a complete explanation on #MedMastodon.

(Btw Iā€™ll posting answers sooner along with bonus content there from now on)

Mastodon is good, but Metatext is better IMHO because it similar to twitter

3/x open the app and ā€œsign upā€. Here you will pic a server. For most #TwitteRx folks thatā€™s med-mastodon.com. Search for it and give it time to ā€œpop upā€.

Then create your email and password.

Thousands have made the jump from #MedTwitter to #MedMastodon!

One issue Iā€™ve heard is ā€œI canā€™t get the app to workā€ or ā€œI canā€™t find the serverā€

Good news: there are many mobile apps to choose from. Personally I think Meta-text & Tooot are easier to setup.

Hereā€™s how.šŸ§µ
1/
1ļøāƒ£ Setup a #MedMastodon account:

The easiest way is just follow this invite
med-mastodon.com/invite/JLmsx3H6

You should see something like this.šŸ‘‡

Choose a name (or ā€œhandleā€), enter your email address, & makeup a (secure) password.

Thatā€™s it!

2/
Once you confirm your email you should be able to login through the browser. Done

2ļøāƒ£If you prefer an app, you can choose from many options in the App Store.

Personally, I like Metatext; itā€™s open source, free, no ads, & doesnā€™t collect any user data!

Nomenclature question: what do you call it if someone has an elevated PaCO2?

1/
Medically they are synonyms. Both terms - hyperCAPNia & hyperCARBia - *mean* the same thing: having an increased PaCO2.

But the difference is a really big deal if you are a linguist.

2/
Etymologically, there is a (silly) difference:

Hypercapnia comes from the Greek word hyper (high) + the Greek word kapnos (smoke); both Greek roots šŸ‘

Hypercapnia comes from the word Greek hyper + the *Latin* word Carbos (meaning charcoal); 1 Greek + 1 Latin root šŸ‘Ž

What is #MedMastodon & why might we need it?

A short thread explaining what it is & why I think itā€™s time to consider moving to a free, self-moderated medical community as a #MedTwitter alternative.

1/
Despite its flaws I think Twitter is a great platform for medicine/science.

Itā€™s a fantastic way to follow breaking news & scientific pubs. Itā€™s a great way to hear what brilliant people think. It can be an OK way to engage in debate.

Iā€™m a better doc because of #MedTwitter
2/
Where else can you see the moment impactful research is published? Or hear the authors explain it & discuss with them?

Where else can you hear about the experience of colleagues around the world?

Iā€™ve learned a lot and made some great friends IRL on here.
