2) Optimization is the favorite word for many people, governments or corporation, looking that input and output are "well" used, while everything extra is a "waste" or what is better called redundancy..which end up beign way better for the unkonws (as @paulportesi would stated).
3) This happens everywhere, beign even pathological, specially when not all risk are created equal, some will look at easy to spot risks, but the most terrible and common in our modern times, are the silent ones.

Junawal, de l’Union des paysans, milite contre les monocultures de caoutchouc. Accusé d’avoir brûlé des équipements appartenant à l’entreprise PT Lestari Asri Jaya (filiale de Royal Lestari Utama possédée par Michelin), il est arrêté le 26 mai et est toujours en détention. 
Pour l’Union des paysans indonésiens, «Michelin a fermé les yeux sur la réalité au niveau local, où les entreprises qui répondent à ses besoins de production ont privé les agriculteurs de leurs droits fonciers.»
