Long-standing unwanted & unpleasant genital sensations in a woman of neurological origin

1. 41-year old woman presented with 3-year history of persistent abnormal sensations involving vagina, #clitoris and labia, without #sexual interest or #desire.

2. She found it hard to describe the sensations but used terms like vibrating, tingling, painful, throbbing, pulsating and numbness in vagina & surrounding regions. There was a sense of imminent #orgasm. Sexual activity & orgasm, however, did not relieve her symptoms.
3. Symptoms got worse on sitting and on lying down, but were lesser on standing or walking. Tight clothing or underwear aggravated her symptoms. She also had #pain in both legs, and felt the urge to constantly move her legs to reduce pain & discomfort (in legs & feet).

This is the most straightforward explanation for chronic symptoms in at least a subset of #LongCovid patients. Partly b/c if the #virus is still present its activity can directly contribute to other phenomena also being documented in LongCovid
2/ Persistence of #SARS-CoV-2 in tissue could lead to shedding of spike protein into blood, which can catalyze the ongoing formation of microclots and hyperactivated platelets
3/ Persistence of SARS-CoV-2 can lead to ongoing downregulation of interferon and/or T cell signaling by the virus, creating an optimal atmosphere for activation of EBV or other #pathogens normally controlled by such immunity

In a meeting I watched today, microglia priming was mentioned in #LongCovid and #ME/CFS. It’s important to clarify what “microglia priming” means. Microglial priming does not mean that after a trigger has “cleared” microglia remain perpetually activated
2/ Instead, microglia priming goes like this 👉 When microglia or other glial cells detect #infection, injury, or inflammatory mediators, they enter a state of activation in which they change morphology and release their own neuroexcitatory inflammatory mediators
3/ Then, after activating, they retain a “primed” functional state which causes an even more robust response to *subsequent* infectious/immune/#inflammatory challenges. And as cells, microglia live long lives (they are not replaced as often as many other cell types)

Awesome study used mice + human brain organiod model to show #COVID-19 can infect the CNS/#brain, where it specifically infected radial #glia, neural stem cells + #neurons. The #virus altered the #metabolism of the neurons it infected + cld use neuron cell machinery to replicate
2/ It’s not surprising that #COVID-19 can infect neurons given the nasal cavity is connected to the #CNS via the cribriform plate, olfactory epithelium + #nerve. This led the team to state that “the brain should be considered a high-risk organ system upon #respiratory exposure”
3/ The team also showed that, at least in some #COVID-19 patients w/ #neurological symptoms, there is a robust antibody response to the #virus in the #cerebrospinal fluid (CSF). In fact, CSF-containing antiviral #antibodies blocked COVID-19 infection in brain organoids

Cool new study which found that a range of #pathogens (some capable of long-term persistence in tissue + #blood) upregulate expression of the cell surface protein CD47 in the host cells they infect as an #immune evasion strategy
2/ I fact, upregulation of CD47 is a very smart survival strategy by such pathogens, b/c it interferes w/ the host innate immune response that is normally supposed to identify, target and kill them!
3/ Specifically the team found that upregulation of CD47 by the pathogens under study slowed uptake of dead/#infected cells by the #immune system, including the downstream activity of #antigen presenting cells that are needed to recognize pathogens in the first place
