Today's urgent and emergency care recovery plan - a quickšŸ§µ

TLDR: Pragmatic set of measures to improve flow and ease UEC pressures, but underpowered and too focused on finding short-term fixes for long-term problems
Ambulance services and A&E departments are under huge pressure, leading to longer response times, delayed handovers and long waits.

Behind the numbers are patients left in pain, enduring unnecessary suffering and, in some cases, lives tragically lost.

Fundamentally, UEC performance is about patient flow into, through and out of hospital - not just what happens in ambulances or A&E

šŸ“ˆin response times, trolley waits and delayed discharges all point to problems with flow

Granted this is not the most exciting of reading, but essential for anyone who may:

šŸ‘‰ find themselves, or a member of their family in a NHS (England) hospital from 1st September

šŸ‘‰be responsible for supporting discharge

(Thread)
gov.uk/government/colā€¦
Few things to note in this:

Emphasis is heavily on rapid discharge & community assessment: moves most assessment post acute care into communities-piling pressure onto local authorities & CCGs

Free support available on discharge up to 6 weeks šŸ‘‡
The 6 week cut off in communities will become the new point of critical failure in system flow while agreements are reached about who

For people with complex needs this has been a historic battle ground of dispute and delay with inadequate CHC assessment capacity šŸ‘‡

THREAD: Last couple of days to comment on the #primarycarenetworks services specs. Lots of feedback to date that tells us we need to make changes. We are committed to doing just that @NikkiKF @NHSEngland. You can help us with feedback: engage.england.nhs.uk/survey/primaryā€¦ 1/9
There are some really important points about this work:

These are draft and this is a proper, comprehensive and dynamic engagement exercise, being conducted "at scale". The feedback that we get will influence the final versions that we negotiate. 2/9
We cannot afford to make them over-ambitious: we would like PCNs to welcome the final versions as sensible expressions of ambition. CDs and #generalpractice teams should feel the final versions are deliverable and that they are aimed at the right areas 3/9

Iā€™ve been through todayā€™s @CareQualityComm #StateOfCare report so you donā€™t miss the bits that matter but donā€™t make the headlines. Hereā€™s a link to the report.

Buckle up for my annual thread. 1/11 Front cover of CQC report on the state of health care and adult social care in England 2018/19.
2/11 #StateOfCare identifies the challenges faced by people with multiple needs and describes people and their families needing to chase services to get the right care. @RichmondGroup14 work on multiple conditions has also found people feeling exhausted and overwhelmed. Page 15 of CQC report.Page 18 of CQC report.
3/11 #StateOfCare shares a story from @RichmondGroup14 member @age_uk that shows how people can sometimes be viewed by professionals with a narrow focus, especially outside obvious moments of transition. Page 19 of CQC report.

A new perspective on the old question of ā€œdoes competition law apply to the NHS?ā€ from @NHSEngland @NHSImprovement #NHSLongTermPlan proposal to remove @CMAgovUK and @NHSImprovement concurrent powers (=parallel jurisdiction) [a thread] (1/7)
@NHSEngland @NHSImprovement @CMAgovUK ā€œā€¦NHS Improvement would no longer have general competition law powers to enforce the Competition Act 1998, or to conduct market studies or make market investigation references under the Enterprise Act 2002ā€(para 54, bit.ly/2lTLYFk). First CA98, then EA02 (2/7)
@NHSEngland @NHSImprovement @CMAgovUK @CMAgovUK power to use CA98 to cases involving NHS is already curtailed by Regs 5 & 8 Competition Act 1998 (Concurrency) Regulations 2014, so only @NHSImprovement appears able to intervene. (3/7)

British Social Attitudes #NHS + #SocialCare satisfaction data analysis by @TheKingsFund @NuffieldTrust @RuthRobbo @jappleby123 @HarryAEvans @NinaHemmings92 is out. Wonā€™t rehearse content but thread gives view from @HealthwatchE perspective. @danwellings @jacoblant #HealthForCare
Hereā€™s the link. And my subsequent comments on the #NHS figures are made against the background context that although thereā€™s a statistically significant decline in overall satisfaction, which is concerning, itā€™s still quite high for individual aspects.

Top 5 things causing #NHS dissatisfaction are all at the heart of what the #NHSLongTermPlan focuses on - so getting local/regional plans right is vital and the @HealthwatchE networkā€™s current engagement work in partnership with @NHSEngland is so important.

Wide press coverage this morning about @NHSEngland plans for the 4-hour A&E target, with @NHSEnglandNMD quoted on how @HealthwatchE and others are informing this work. Weā€™ve been saying for a long time that there needs to be a new approach to targets. This short thread says why.
When @HealthwatchE fed 85,000 peopleā€™s experiences into #NHSLongTermPlan development, this included 6,500 experiences of A&E. We gave our analysis to @CommonsHealth for its inquiry on LTP. The select committee has now published our evidence. Hereā€™s a link.

So some interesting stuff coming out of @NHSEngland's board meeting today (well, interesting for those of us with a passion for procurement......). Seems like the beginning of a move by against outsourcing in the #nhs to accompany the #NHSLongTermPlan /1
"We are concerned that the comparative weight that current legislation gives to competition will act as a drag on efforts to improve collaboration between NHS bodies and provide integrated care" #NHS #reformhealth bit.ly/2Vqizyn /2
Then goes on to call for the removal of article 75 Health and Social Care Act (which gives the CMA a role in observing NHS transactions) and removing NHS commissioning from the Public Contracts Regulations - all in efforts to make #NHSLongTermPlan feasible /3

Mini-thread on bullying & an even bigger problem. I wrote this on bullying in the NHS & #metoo for @NuffieldTrust reflecting on last year's #ntsummit. nuffieldtrust.org.uk/news-item/viewā€¦ Image
Two parts to the argument: bullying way too high, and not sufficiently part of the mainstream policy conversation. Idea was whether we could learn from #metoo in terms of calling out woefully unacceptable behaviour.
Where are we a year on? Not great. From #NHSStaffSurvey bullying metrics ā¬†ļø across the board. Not massive, but against a backdrop of everyone thinking they were too high to start with, that's not good. nhsstaffsurveys.com/Caches/Files/Sā€¦ Image

As we begin 2019, I want to give a shout-out to our great NHS - and all the inspiring staff that keep it running during the holidays and throughout the year. Last year the NHS turned 70. We must fight to secure its future for the next 70 years and beyond. (1/)

#NHSLongTermPlan
The Tories have wrecked and ravaged our vital health and social care services for 9 years. Their ā€˜10-year planā€™ is a sham. All the numbers add up to is disaster - insufficient funds, inadequate staffing and lack of foresight.

The Tories just canā€™t be trusted with our NHS. (2/)
It has taken a government obsessed with slashing budgets and cutting corners, a government hell-bent on imposing deadly and devastating levels of austerity, a government slammed by the UN for its unfounded cruelty, for cracks to appear in our mighty NHS. (3/)

"THE GOVERNMENT MADE ME PREJUDICED!", cried Sarah.

I was in Clacton, in a living room with Sarah, brother Sam & daughter Linda, small children & a few pets. All three adults voted Leave & UKIP.

This #TuesdayThoughts, I want to share bits of their story with you.

#phdchat
How did that happen? I asked Sarah.

"There are so many English families living in one room, yet they're letting all these in and they're housing them, having houses built for them!", said Sarah.

Who is being housed, I ask? Have you been waiting for a long time?
Brother Sam cannot walk after an accident and is unable to work. Sarah has a lung disease and also has been out of work for a few years. It's only Linda who works in the household, who has two small children and no qualifications. She's working on a vocational qualification now.

Lots of good stuff in the #NHSLongTermPlan, especially if you're a student of the history of personalised care
Personalised care is up front, in chapter one: "People will get more control over their own health and more personalised care when they need it" #NHSLongTermPlan
Personal health budgets continue to be the significant, transformational change led by people themselves: we will go from over 32,000 now to 200,000 by 23/24 (para 1.41) #NHSLongTermPlan

If you love the NHS more, how do we best protect it?
We can protect it as an EU member. Brexit makes it vulnerable to people like Trump.

But they're saying more money is coming to the NHS. Do the numbers add up?
