Almost 25 years ago I entered the world 🌍 of #raredisease when 1st my husband was dx w #PRSS1 & then our children.
What I’ve learned is #HealthcareInAmerica is brutal & often #harms those most in need.
The system is broken.
There is no disguising it.
I believe that our 1
#Healthcare system truly only benefits a small populous; individuals that are mainly #healthy & don’t belong to a marginalized group.
There are monumental system issues that need to be addressed that impact #medicalprofessionals ability to provide necessary 2
care. I understand that. I believe that #patients, #caregivers #medicalprofessionals need to work in collaboration to push back on the systemic issues that plague the system. I absolutely can give & find grace in many situations.
What I have no time for is the void of 3/

#NPSADay is on Saturday. How will you get involved this year? Start by learning the six actions you can take to prevent #physician suicide at NPSADay.org. #NPSADay

@drbreenheroes #FirstRespondersFirst
Action 1 is to learn the Vital Signs. Prevention begins with understanding suicide warning signs. The HEART acronym helps to easily remember the signs: Health, Emotions, Attitude, Relationships and Temperament. Learn more at NPSADay.org. #NPSADay
Action 2 is to share suicide prevention resources. Physicians face many challenges, and there are resources that can provide immediate and ongoing support. Help #physicians by spreading awareness of these resources. Learn more at npsaday.org/share-suicide-… #NPSADay.

Heartening to know that today there are gobs & scads of young #Dentist's who want to learn more about who precisely WAS "C.C. Bass, M.D", and what did he have to say about #PreventiveDentistry?
For the last few decades or so, just about any young #Dentist, when they first learn the real story of who "C. C. Bass, M.D." is, and what he actually did, are puzzled.
A couple of channels results from their #DentalSkool teaching, and thus their thinking.

#chronicpain & are young adults. Our kids 1st 13/16 years of life, our family had a #safetynet, their #pediatrician. He was highly intelligent, compassionate, he challenged the system & in the most authentic sense #partnered with families. #Pain was not a major concern when 2/

"Need to effectively utilize #CNMs has never been greater," says Kohl from midwife.org

Finding a supervising provider may prove difficult.
Body of evidence has consistently shown that although these barriers do not improve care.
Kohl:
"Time to follow the economics and evidence and remove supervision of practice"

Notes that team based care is the "future of #healthcare in the US"

Also, at midcareer female success is threatening to men our age AND to younger men who rely on patriarchal patronage systems. Women, overpoliced by the system, then, yes, are tougher bosses AND we *do* tend to adhere more closely to ethics.

I do do all:
On #MedTwitter there is a pattern of “callouts”, yes vs all genders & ages, but also pattern of misogyny and misogynoir. It happens towards nurses & doctors alike. One of the reasons I connected with @ShrimpBrokkoli is because I saw a yt male resident ridiculing her online.
I saw that behavior being taught by NON-clinical faculty & that REALLY crossed a line.
If you are a woman + nurse + frontline + Black/WOC, you already live with so much violence towards you. Added online made up drama from a trainee vs nurse?
No, no, & no.

This is what targeted racial harassment looks like: nitpicking things like “smiled” or “wore clothes like a celebrity, as a celebrity” or “bought cookware.” It is part of the “gold digger” narrative.
This is common in #healthcare#professionalism” where there is hazing & bullying by racist &/or classist made up non-rules lacking standards…yet people are called “breaking the rules.”

It’s exactly how bullies operate: double standards, mislabel/smear, exclude, marginalize.
In the U.K., “BAME” includes the formerly colonized, thus making race constructs or power different from U.S. as latter had race-based chattel slavery.

Thing is, those formerly colonized have many who have internalized self hate & racism.

the ‘care’ they receive has often been #traumatic as they don’t fit nicely into a box 📦. It’s because of their experiences (& my husbands) that I advocate. As young adults they still have to interact w the #healthcare industry but they are distrustful ( & rightly so). So many 2/

I wanted to share some comments by #parents whose children have #pancreatitis. As I have shared before, the #pain care for these kids is NOT adequate. Their #pain is often not being addressed. I have asked parents to start writing up their stories to share. When #pain is not 1/
addressed it causes trauma, not only for the child/youth but parents. We know unaddressed #pain can lead to #chronicpain. I absolutely believe in #biopyschosocial approach but what we are seeing is a #psychosocial approach being utilized and #bio aspect missing. This leaves 2/
#parents and #patients feeling angry & abandoned. One of the reasons I started getting involved in #advocacy is that my kids #pain was dismissed, they were labeled. This leads to #trauma, #chronicpain & distrust of #physician. What HCP need to understand is that when this is 3/

In light of the Boston police Union leader child abuse, this is very plausible, even probable. I have 2x, on behalf of someone else, reported, via Title IX - the woman or gay man was not feeling safe enuf to report. Yes, there is blowback when one reports. But we can’t be silent.
I am a mandated reporter as a #pediatrician. I *must* report for my patients or face legal consequences. Am same for staff, students, trainees. But in #EmergencyMedicine who do I call if a child may be unsafe at home? Sometimes, cops. Are cops safe?

boston.com/news/local-new…
I have seen #mentalhealth and entire life paths destroyed by abusers preying on children like: This happened to too many people I know. I have seen institutions intimidate, mislabel, bury stories of the victims. (Some predators wait until 18th birthday.)

Further, suboxone has required special training and certification, adding an administrative barrier and creating an added fear barrier among risk-averse clinicians whereas oxycodone can be prescribed easily with the DEA certificate we all have.
Because doctors/prescribers are definitely lacking in administrative burden and CME obligations, we're just twiddling our thumbs eager for someone to ask us to get MORE educated, taking time off from patient care to get this:

The law enforcement side sees doctors through this lens. So doctors seek to limit their risk and liability. I'm all for taking down those who harm patients. I've testified in cases like the ones below for the AG's office as a medical expert.

The folks who are anti-diversity need to update their tired playbook.

Sharing space for win-wins is not the same as hating people in a space.

Violating professional ethics by trying to diagnose via Twitter only damages own professional reputation by displaying poor judgment.
The time to have diagnosed by Twitter was here - instead of holding leadership accountable, people scrambled to put guardrails and install people to be "adults" in the room to reassure the public.

Actually don't be reassured.
Do something.

theconversation.com/thousands-of-m…
This was time to raise alarm, not "reassure"

All of you who distribute pacifiers and those eager to be pacified killed Americans

Plus, there was utterly bad and WRONG advice from a #physician and #publichealth #leader that "no need" to think about DNRs.

I was discussing #healthcare cost variation (& #EHP #RBP models) across geography with a colleague earlier today. I have followed the work of @zackcooperYale and thought it would be interesting to, in very simple terms, visualize some of the cost variation. A short thread. 1/n
2/n I started looking for a datasource. While our #benefitconsulting and #insurance clients engage networks of private insurers, that data is subject to different rules... I wondered what variation "looks like" for Medicare claims. I landed here: dartmouthatlas.org
3/n First, there is a ton of #Medicare data. It can be downloaded by state (see below) as well as to a county level (which I pulled into Python.) To consider the amount of cost attributable to different segments, I looked at the largest categories with observations by county. ImageImage
