A week ago I completed my third week of #COVID19 inpatient service over a five-week period. I have been reflecting on this experience and wanted to share a few clinical insights and a great deal of gratitude. I'll start with the gratitude! 1/9
1st- thank you to @JBPritzker & @LoriLightfoot for their polices to #FlattenTheCurve allowing hospitals to have beds/resources needed for patient care. Thanks also to early/ongoing efforts of local advocates for #stayathome policies! cc @futuredocs @LJoyZ_MD @ShikhaJainMD 2/9
2nd- thanks to all that helped acquire & distribute #PPE. One of my greatest fears was no/limited #PPE during my laters service wks. This fear has not been realized thx to incredible efforts by: government (@JBPritzker & @LoriLightfoot), @UChicagoMed, & local #PPENow leaders 3/9
Did Feb20 #COVID19 Scare after UK Bus Industry Conference at Westminster serve as warning to UK BusCo Executives to immediately conduct Risk Assessments & take Protective Measures like #PPENow to protect Bus Drivers? AFAIK @TfL did nothing until APRIL bbc.co.uk/news/uk-515032…
AFAIK @MayorofLondon instructed Bus Contractors to take protective action ONLY AFTER Bus Drivers started DYING and started making noise on Social Media!!!

Stopped for a sandwich a few days back & started conversation with guy in queue. He is an importer/supplier of #PPE.
He told me he has tried several times to make contact with government & never received any response despite having a large amount of #PPE in stock./1

This #PPE supplier was contacted by the Health service (HSE) in Dublin recently & sold them a large quantity of #PPE after just one quick call and an emailed photo of the pallet of stock. Again the UK government did not respond./2
This #PPE supplier sold stock to two intermediaries who sold some equipment on directly to 2 NHS Trusts at a profit. His further attempts to offer stock to local & national government/Trusts directly were unsuccessful./3

Maybe if we had an expert Executive Branch Pandemic team coordinating a response they would've strongly advised Pompeo NOT to collect donations of 17.8 tons of critical Medical #PPE & ship it OUT OF THE COUNTRY over a month AFTER the CDC warned this pandemic was coming to the US.
And of course after the US donated tons of medical supplies to China, we see China turn around & sell $467 MILLION in PPE equipment that we in the US DESPERATELY need to SPAIN despite the fact that our State Dept's now desperately trying to buy PPE overseas cause we need #PPENow
I lost my best friend to Covid19 a few hours ago. @DrFrankGabrin was an ER Doctor in NYC. He planned to go back to work when he recovered. It took only five days from the first sign of symptoms. He leaves a husband of less than one year behind. We are devastated. #PPENow
2) Symptoms developed 9 days after he’d been forced to use the same mask for multiple shifts. He was told to self quarantine. He woke up this morning and couldn’t breathe. In less than two hours he was gone. He never made it to a hospital. His husband is sick too. #PPENow

#POCUS in #COVID19 - there is some controversy about its use- due to infection control issues. IMHO - in EM - the role is even bigger in undifferentiated dyspnea in this pandemic. How do you know if its COVID19 or even pneumonia in the beginning!
The EM question is: Can #POCUS add value to physical exam in Acute Dyspnea performed with full #PPENow - in resource limited setting? I think the question answers itself.

Anosmia. The strangest & most fascinating symptom of all. Discovered it when the brand new diffuser wasn’t working despite dumping several drops of strong scented oil in. Then a friend asked “so is that anosmia thing real?” Then it hit me. 3/
1/4. The H1N1 (influenza) virus killed an estimated more than 50 million people in the period 1918-20 worldwide. We have to take Coronavirus seriously! The protection of Health personnel is paramount to the containment of the Coronavirus pandemic. #PPEnow
2/4. There is need for re-organisation of systems in place. We need hotline numbers that members of the public can call on for assistance. The sick will always rush to emergency centers in times of crisis unless there is alternative information readily available.
3/4. Dedicated ambulances with fully clad crew in protective gear should be in place to attend to suspected cases in their self quarantine environments or homes and move them to treatment centers. This way the sick would be given an alternative to going to their usual hospitals.
