First, instrumental variables and causal graphs:

An instrument is a special type of variable that is associated (causally or otherwise) with the exposure you’re interested in.

The directed acyclic graph (DAG) below shows the basic required structure.
There are three key requirements for a variable to be an instrument:
•it is associated with the exposure
•it is not associated with the outcome EXCEPT through exposure
•it does not share any causes with the outcome (ie unconfounded).

Proud to have led this #clusterRCT, presented @ESCCardio's #LBCT session & published today in @JAMA_current. Among 2495 adults (50% women) hospitalized for #HF, a transitional care model implemented in our publiclyfunded system did not ⬇️time to comp readmission/ED visit/death.
There was a significant improvement in the patient-reported outcomes of #discharge-preparedness, #qualityofcaretransition, and #QOL. However, there was no improvement in #QALY. The #PROs were not adjusted for multiplicity, but we will do post-hoc exploratory analysis.

1/ Another reason not to trust Scheer and the CPC.

The Journal de Montréal reports that birth tourism is growing in Quebec too.

But the CPC no longer plans to do anything about it even though delegates voted for that at the Halifax convention.
2/ "There is clearly abuse, but I do not think it's a pervasive issue at the national level," says Michelle Rempel, the Conservative MP responsible for immigration.

She argues “more serious analyses are needed to know the extent and causes of the problem.”
3/ So, once again, CPC will do polling, focus groups, studies, and wait to see where the wind is blowing instead of taking a principled stance on an issue as simple as birth tourism, as it did with the Global Compact of Migration.

If we’re going to talk about #pragmatic trials, then we need to start with a definition. The simplest definition I’ve seen is from a trialist I interviewed for my recent @JClinEpi paper (…)

pRCTs “...attempt to address effectiveness in real world settings”
Sure, it’s a bit vague but it’s useful because, like Tolstoy’s unhappy families, #pragmatic trials are all different.

So, what makes me think #SCOTHEART is pragmatic?

My 2¢: lack of blinding, inclusion criteria based on symptoms not diagnoses, & standard care as comparator.
