Can you monitor maternal & fetal safety in clinical trials in pregnancy? Now you can, with #MFAET version 1.0 published this week @ISPDHQ
The 12 maternal & 19 fetal Adverse Event definitions are adopted by @MedDRAMSSO
obgyn.onlinelibrary.wiley.com/doi/10.1002/pd…

1/ Researchers @ucl @UCL_IfWH @UCLHresearch have developed a standard terminology for safety monitoring for pregnant women and their fetuses during development of drugs for pregnancy. #Pregnancy #AdverseEvents #MaternalSafety #fetus #MedDRA #MFAET

2/ Adverse Event (AE) monitoring is central to assess therapeutic safety. The lack of an AE framework in pregnancy trials severely limits understanding risks in pregnant women. #MFAET was developed by international consensus of healthcare professionals & patient public groups.

New research from @JohnsHopkins about maternal health!

Vaught AJ, Minhas A, Boyer T et al. Incidence of essential hypertension but not echocardiographic abnormalities at four years with a history of preeclampsia with severe features.

In this new study, we followed up with women four years after they were diagnosed with preeclampsia during pregnancy. What type of research study is that?
#ph260720 #Epidemiology
Preeclampsia is a pregnancy-specific disorder that causes high blood pressure and evidence of damage to the kidneys and/or liver. Preeclampsia and other hypertensive disorders of pregnancy impact between 5-8% of births in the US.

Q-Do kidney diseases affect women differently?

Women have
⬆️Autoimmune diseases (SLE, RA, Systemic Scleroderma)
In #Women of Color that's 2- 3X commoner, early-onset with⬆️ complications

⬆️UTI due to short urethra
In #Pregnancy ⬆️risk of pyelonephritis and sepsis

Excited to see our paper in @JCI_insight! Silver lining of lab shutdown this yr was time to wrap up this project which has been yrs in the making.

We studied a Stox1 KO mouse model to probe mechanisms underlying hypertensive disorders of #pregnancy. 1/
insight.jci.org/articles/view/…
STOX1 is a TF and predicted #preeclampsia risk gene IDed through family-based genetic linkage studies (van Dijk, @NatureGenet 2005). A variant in the DNA-binding domain was proposed to alter protein function ➡️ preE susceptibility. But LOF model had not been studied. 🐁 2/
*We found that loss of Stox1 ➡️ gHTN assd w placental hypoxia + abnormal ECM deposition.
*We observed activation of the uteroplacental renin-angiotensin system in Stox1 KO mice.
*Treatment w an ARB, losartan, rescued the gHTN phenotype, validating the mechanism. 3/

1/ THREAD: #COVIDー19, Pregnancy, and #ACE2
A thought has been tugging at me for a few days. Currently, ACE inhibitors are pregnancy category C in the 1sr trimester and category D in the 2nd and 3rd trimesters.
#SARSCoV2 #COVID2019 #Covid_19

2/ Teratogenicity, 1st trimester exposure to ACE inhibitors, and fetal anomalies/congenital malformations has been debated. Most malformations identified were various cardiac septal defects.

3/ That being said, 2nd and 3rd trimester exposure to ACE inhibitors is associated with oligohydramnios, hypocalvaria, anuria, renal failure, neonatal hypotension, and patent ductus arteriosus.
