1) Every doctor who administers a vaccine must sign an attestation that they have READ THE INSERT and understand the material they are handling and administering.
2) Every doctor will PROVIDE THE INSERT to patients/families prior to vaccination appointment.
3) Every doctor will sign an attestation stating they understand the product and potential risks and are confident in performing the vaccination with all the integrity of their oath.
