Thrilled to share our latest article in @NaturePortfolio Reviews Immunology exploring the role of platelets in #immune mediated inflammatory diseases such as #SLE. Open access link 🔓 rdcu.be/c4ijW.
A thread
In autoimmune & inflammatory diseases, platelets get activated in response to circulating autoAbs (directly or through FcGR2A binding), soluble factors such as DAMPs or infectious agents (ie through platelet TLR7). Platelet activation often follows disease activity -> biomarker
Upon activation platelets release pro-inflammatory molecules (ie, IL1b), express glycoproteins that promote the interaction with immune cells (ie P-selectin or CD40L) and release plat-derived extracellular vesicles which enhance the inflam « signal » thorough body & organs.

Back in the days of John Humphrey Noyes before he got suspended for posting a study indicating there's viral persistence in epithelium of kids, because everyone includes kids.
How does a twit like this get 108K views propagating “what if viral persistence is REAL”?

It’s almost like we are back to banning twitter accounts for insisting SARS is airborne!
WHEN the rest of non-SARS experts acknowledge that “viral persistence is a thing”, we will be one step closer to acknowledging the true immunity dysfunction threat of SARS

🧵 Short summary thread on our recent publication looking at the impact of Anti-Rituximab Antibodies in #SLE with our recent manuscript published in @RheumJnl

This has been a story that we have been looking at for a few years now

💻 Link - academic.oup.com/rheumatology/a…
In spite of failing to meet primary endpoints in the LUNAR and EXPLORER trials, a number of Open Label studies have shown benefit of B cell depletion with Rituximab in #lupus

In England, we have access to the drug for severe and refractory cases 💊 💉

england.nhs.uk/wp-content/upl…
During my time at @uclmedsci I was interested as to why patients receiving rituximab for #SLE occasionally developed infusion reactions to treatment, whilst I did not see this in patients receiving the drug for rheumatoid arthritis 🤔

Q. What up doc. My #IBM patient doesn’t have #NT51a antibodies and my #SLE patient has #NT51a antibodies. I’m confused.
A: If only life wasn’t messy. 30 percent of #IBM patients don’t have #NT51a and false positives happen with #SLE and #Sjogrens which can cause #overlapmyositis.

Hi hi hi. We see lots of red flags on a daily basis when we review labs.

One thing we see commonly is #lymphopenia. Should we care? When can we dismiss it and when should we work it up?

Let’s find out.

2/
- Lymphopenia assc w/ ⬆️risk of sepsis
- More common in elderly and w/ co-morbidities
- lymphs account for 20-40% of WBC, may go unnoticed when WBC checked w/o diff
- Primary- recurrent infxns in kids, CVID in adults
- Secondary-⬇️production; ⬆️destruction; hiding in tissue
3/
What is the most common cause of lymphopenia in hospitalized patients?

Nice work @drmfreire + team at @JCVenterInst! 👉 They found that local #periodontal pathogen enrichment was positively associated with high systemic inflammatory profiles in patients with Systemic #Lupus Erythematosus: biorxiv.org/content/10.110… ImageImageImageImage
Quote from paper 👉 “...the gut #microbiome, and recently the #oral microbiome, showed direct impact on SLE subjects, the complexity of chronic diseases such as #SLE is beyond isolated body compartments, and it requires the integration of host-microbial interactions.”
Also 👉 “At the center of #autoimmune pathogenesis, this study provides evidence that #inflammatory response to #microbiome controls the severity, and magnitude of the #SLE disease”

Important @jonykipnis statement 👉”Recent advances have directed our knowledge of the #immune system from a narrative of “self” versus “nonself” to one in which immune function is critical for #homeostasis of #organs throughout the body”:ncbi.nlm.nih.gov/m/pubmed/30504… Image
IMO, this shift away from “self vs. non-self” extends to “#autoimmune” disease 👉 Here, I explain how “autoantibodies” in patients w/ #SLE, #MS etc are likely created in response to persistent #pathogens (and react w/ human tissue via molecular mimicry):discoverymedicine.com/Amy-D-Proal/20… Image
It follows that science + #medicine + #pharmaceutical companies must move towards a new #paradigm 👉 ...in which the human #immune response is no longer treated as the enemy, but is instead regarded as our strongest ally in maintaining #microbiome + system-wide #health.
