Statins act by reversibly and competitively inhibiting HMG-CoA reductase, a key enzyme in the mevalonate pathway of which cholesterol is a final product.
Cholesterol acts as an intermediate for steroid hormones, bile acids and vitamin D, CoQ10 and is crucial to the integrity of all cell membranes.

This partly explains the plethora of statin-associated symptoms (SAS) that manifest in up to 30% of patients.

Too often, statin advocates will communicate a drug’s relative risk reduction (impressive number), without quoting the absolute risk reduction (trivial number), a practise we consider unethical (see graph)
The FOURIER trial was cited as evidence of benefit but it did not show a mortality benefit. In fact, there were more cardiovascular deaths and total deaths in the group taking the PCSK9 inhibitors compared to placebo, albeit not statistically significant.

11) It’s #Transcytosis (an active process), through a vesicular pathway involving #caveolae, scavenger receptors (#SRB1) and activin like receptor kinase 1 (#ALK1). Hence for a given blood level of LDL-C the amount of atherosclerosis is variable.

2| Let's start with MI def. Billing codes less accurate than adjudicated MI outcomes, yes? Could be nondiff misclassification which bias to null. But could also be diff w/ ascertainment bias (look harder, anchoring) knowing CCTA result, since 2/3rds normal or nonobstruct.
3| It is conjecture. But basis of clinical reasoning is to factor in test results. The posterior probability for this theory moves up much higher when we try to figure out by what mechanism did a diagnostic testing strategy lead to better outcomes, since the radiation did zilch
