Do anti-seizure medications (ASM) prevent late post-traumatic epilepsy?

1. Traumatic brain injury (TBI) is unfortunately a common cause of disability and death. Seizures after head injury can adversely affect the outcome of patients with TBI. Does early ASM help?
2. A recent systematic analysis looked at the possible role of ASM in preventing early & late post-traumatic seizures, and in reducing the risk of death.…

3. The risk of early post-traumatic seizures (occurring within the first 7 days after injury) was significantly reduced in the group taking anti-seizure medications. Image

The blood-brain barrier (BBB) is a crucial defense mechanism for the brain, regulating the exchange of molecules between the bloodstream and the brain. When the BBB is disrupted, it can have severe consequences for brain health. (1/12)
Alzheimer's disease is one example of a condition that is exacerbated by BBB disruption. When the BBB is compromised, amyloid beta-peptide deposits can accumulate, leading to #inflammation and disruption of neuronal function. (2/12)
A disrupted BBB in #Alzheimers means astrocyte and microglia inflammation, disruption of synaptic transmission, and myelin destruction. (3/12)

📢 4yr Acc-iversary 📢🧠🤕

I love movement! About 6 years ago 🤷‍♀️ I fell in love with @CrossFit methodology.

I decided that as well as working at MacBlair, I wanted to be a CrossFit coach. Weekend of 10/11 of Nov ‘18, I was attending a CrossFit Level 1 course in Image
Manchester, to get my coaching qualification (I still passed the course).

It is difficult for me to explain my accident; if you know gym racks 😆 I sustained a concussion and whiplash when I accidently initiated a movement with my legs instead of my shoulders and drove my
head into a three-bar pull up rack (wasn’t a dirty south bar).

The picture 👆 was about five hours after I did it. Not bad eh?!

👇 is why @CrossFit should have stopped me from continuing with the course but that is a whole other part of my story.

1/🧵 This #Trauma #MorningPaper is a retrospective study of 284 cases of traumatic cardiac arrest (TCA) in Sweden, mostly men with mean age of 38. Are our attempts always futile? @SMHMajorTrauma @nottsurgeon @ShehanHett @karimbrohi @londonsdrcosmo @e1v1m1…
2/ The study region has no #REBOA or prehospital resus thoracotomy services. 90.1% of TCA were prehospital with 64.8% from blunt injuries. Only 6.7% had shockable rhythm.

29.9% survived to ICU with mean days on ventilator of 3d. Only 10% survived to 30 days (GOS 3 or 4 mostly).
3/ In-hospital TCA had a much higher chance of survival (42.9% vs 7.0%) - but remember there is no prehospital thoracotomy/aortic compression/REBOA services, therefore I'm not sure how applicable this data is if setup were different. @viseshsankaran @traumakhan @WilsonMSJ

Sometimes something so cool comes out I can't wait until a Live Brain Fog Recovery Program Lesson to share some of the scientific reasons for doing the program! So here it goes... 🧵
In a recent study on the diet-microbiome-gut-brain nexus in acute and chronic brain injury, findings have shown that the microbiome is affected by neurodegenerative conditions and brain injury states.
While the specific changes to the microbiomes of people suffering from different neurological disorders vary, there is a theme seen in both acute and chronic brain injury. And that theme is that pathogenic and opportunistic bacteria increase and beneficial flora becomes depleted.

If you care about Traumatic Brain Injury, read this:
2.5 million people in the United States sustain a traumatic brain injury (TBI) each year.

TBI has been regarded as one of the strongest environmental risk factors for #dementia, and for #AlzheimersDisease in particular.

Surgical Decision Making Heuristics 🧵

I estimate a #neurosurgery on call doubles the usual 3000 decisions we make each day.

A "razor" cuts away what is unnecessary and simplifies decision making.

Here are 10 surgical razors to sharpen your decisions: Image
Occam’s Razor: (The OG Razor)

"Plurality must never be posited without necessity." If the patient with the enlarging CSDH has worsening falls, its likely the CSDH and not the new medicine they’ve started. Image
Hickam’s Dictum:

"A man can have as many diseases as he damn well pleases.” The patient with the enlarging CSDH may also have had a stroke causing their falls.

Occam's > Hickam's in my experience. Image

Today I am sad. And I am angry. Let me tell you why. A thread (it turns out this was not the last thread I will do, nor, unfortunately, was it the last thread on this issue). 1/15

and who have had repeated concussions or a significant #TraumaticBrainInjury (#TBI).… He is 41 years old, the same age as me. 3/15

Finishing this book presents a major milestone in my life. You see, approximately 6.5 years ago I was found in a pool of my own blood in a parking lot. I ended up in the ICU with a traumatic brain injury. Here’s what I did to reclaim my cognitive function - a 🧵 1/n
The thing about brushes with death (my particular TBI has a ~90% mortality rate) is that they have a powerful clarifying function. Some of the initial thoughts I had was that I :
a - wanted to achieve fluency in French
b - wanted to really embrace Infosec

There was a small problem though. I was dumber than a bag of hammers. After about six months of recovery - I could barely read a children’s book. It’s not that I didn’t know the words. It’s that they all came at me at once.

Proud to announce the final paper of my dissertation is out @ASHAJournals 🎉

Here, we aimed to assess longitudinal performance in language and other cognitive domains important for academic success in young adults with acquired brain injury (ABI) participating in intensive rehab Image
22 young adults with ABI (#stroke, traumatic brain injury (#TBI), tumor, encephalitis) attended ≥ 12-week semester of the Intensive Cognitive and Communication Rehabilitation program. The program (lectures, individual therapy, tech. training), was held 6 hours/day, 4 days/week. Image
14 young adults with ABI served as deferred treatment/usual care control participants. All participants demonstrated language and/or other cognitive impairment on a standardized assessment battery (WAB, RBANS, SCCAN, DCT) administered before/after each semester.

It's #MentalHealthAwarenessWeek and i'd like to talk about the role of #SLT in mental health for people with #TBI and #FND. I could speak specifically to the role of SLT in mental health settings as i've previously worked in many but i want to focus on my current client groups
Increasing insight in #TBI recovery can negatively impact mood. Mood and emotions are definitely the remit of neuropsychology but all in the MDT can adapt to support confidence, self esteem and mood 1/n
I use as a basis for my video work with my clients with #TBI and #FND as the focus is on all the super stuff my clients are already doing. VIG has a strong evidence base, and we know clients learn best in a positive environment 2/n

@US_FDA approved the #Qcollar jugular compression device to "protect athletes' brains during head impacts."

This decision was based on VERY FLAWED DATA and does NOT demonstrate the safety or effectiveness of the Q-collar.

Read detailed thread⬇️

Here is the text from the @FDADeviceInfo press release.

If you don't understand the highly nuanced #MRI technique known as DTI, these results sound straightforward and convincing.

THEY ARE NOT. Don't be fooled by these numbers!🤔

I will dissect these in the thread below. Image

"No significant changes" is based AVERAGE response, not individual.

First, an easy-to-understand analogy below.

If half the sample experiences an increase and half experiences a decrease, they can cancel each other out to falsely suggest "no change" when one does exists! Image

Yeah, remember when #CaptainBoneSpurs said being repeatedly bombed deaf and senseless, or being blasted 30 feet in the air with regularity, isn't really an injury, even if it does make you completely deaf, leave your ears ringing forever, give you #PTSD & #TraumaticBrainInjury.
BTW there's really no such thing as 'mild' #TraumaticBrainInjury. It's a horrific insult for them to call #TBI mild. It's life-changing, but not in a good way. #TrumpHatesOurMilitary #SuckersAndLosers #TrumpHatesOurTroops #VoteVets #Vote! #JoeBiden!…

@UWMedicine we have delivered cognitive-behavioral, psychoeducational, & mindfulness-based interventions for pain, fatigue, and depression via #Telehealth. Sharing tips we have learned along the way...
Over the past 10 yrs, we have conducted multiple RCTs, using telephone & videoconference (groups and 1:1) to deliver our study interventions because of our geography.

Due to #COVID19, we are now moving most of our clinic patients to #Telehealth visits to protect the health of patients, staff, & providers. Many of you are likely doing the same. Here are a few more pointers. @APA @APADiv22 @APADivision38

Today at Davos, the conference for billionaires + others, Trump dismissed traumatic brain injuries (#TBI) sustained by US soldiers in Iraq as not “serious,” and referred to them as “headaches.”

But here’s the thing — they ARE really, really serious.

Why do we say that?

Here’s some of @txtianmiller’s reporting from a *literal* decade ago:
@txtianmiller Even mild TBI can be devastating.

Symptoms include headaches, dizziness, problems w/ memory + reasoning.

Once troops come home, they can have trouble driving a car down the street or comprehending their children’s homework.

This man has no respect for the sacrifices our servicemembers make for our country—he simply has no sense of decency. (2/5)
From @TheBushCenter: #TBI “is an injury to the brain due to external forces which disrupt the brain’s functioning. The disruption may be temporary with full recovery, or there may be long-lasting, even permanent injury to brain tissue”… (3/5)
