In autoimmune & inflammatory diseases, platelets get activated in response to circulating autoAbs (directly or through FcGR2A binding), soluble factors such as DAMPs or infectious agents (ie through platelet TLR7). Platelet activation often follows disease activity -> biomarker
Upon activation platelets release pro-inflammatory molecules (ie, IL1b), express glycoproteins that promote the interaction with immune cells (ie P-selectin or CD40L) and release plat-derived extracellular vesicles which enhance the inflam « signal » thorough body & organs.

I met an old lady who had #schizophrenia. She was on antipsychotics for more than 50 years. Never remitted. Had delusions of infidelity on her husband. He loved her and brought up his kids. The kids are grown up and married now.
She was also a cancer survivor. Docs repeated the same medicines as there weren't too many problems even though she was not fully well too.
I took a bold decision to start her on #Clozapine, a drug with risks but also better efficacy. Discussed with her and family.
They agreed to monitor her blood counts and the process started. She did well. Her delusion resolved. She smiled at her husband after long time. He was very happy to see her change. I was thrilled to see the joy in their family.

Stop terming 'therapeutic' experiences, as 'therapy'. Therapy is structured and focused on the client's issues. There is a particular process for therapy which has fixed steps and actions and challenges.

Therapy involves guidance, insights and support from a professional. Professionals are trained to help you and education is achieved in order to be informed of various methods to provide that help. Not so that the tag of our profession is given to 'things'.

Therapeutic experiences do not necessarily need supervision, guidance, scientific advice or the study of psychology. Drinking tea is 'therapeutic' because one feels relaxed or calm after it. But tea is not your therapist for it to be therapy.
