What is #Pre-Hospital #CriticalCare? A short 🧵:

Also referred to as Pre Hospital Emergency Medicine (PHEM), #PHEM is “that area of medical care required for seriously ill or injured patients before they reach hospital (on-scene) or during emergency transfer to hospital” 1/11 Image
PHEM in Ireland is practiced by a small number of doctors who respond voluntarily to taskings from @AmbulanceNAS to serious illness or injury.

These doctors are supported by charities like @WicklowRR.

Other groups include: @WestCorkRR @EastCorkRR and @CRITICALcharity 2/11 Image
PHEM doctors complement the work of @PHECC practitioners in @AmbulanceNAS @DubFireBrigade and many other emergency agencies.

PHEM doctors can provide additional interventions, medications and decision-making, or simply be the closest resource to a time critical emergency. 3/11 Image

Today's shift has been a rollacoaster of emotions which ended in a very sombre yet stark reminder of the importance of team work, effective communication & most importantly the value of life.#biomedicalscientist #pathology #NHS #Haematology #transfusion @IBMScience

A LONG THREAD:
I started my late shift today at 12pm, I am a Biomedical scientist and I work in the haematology and transfusion laboratories at a busy urban general hospital. I started in the haem dept helping process full blood counts, reviewing results & running countless add on tests.
This is normal for a busy large hospital. I was then asked to pop into the transfusion lab to review my training for manual crossmatches which has now been approved to carry out on analysers. I get as far as starting my crossmatch at 2.15pm, where upon I get a call from

Inspired by @BenMazer and the topic of #platelet #transfusion in #cerebralhemorrhage patients previously taking anti-platelet therapy, I want to take a few minutes to talk about the EVIDENCE in platelet transfusion indications, thresholds, and efficacy. #blooducation
To recap, the PATCH trial (Lancet 2016) concluded that in patients taking anti-platelet medications (aspirin, plavix, etc) who had spontaneous cerebral hemorrhage, platelet transfusion was assc with WORSE outcomes than standard medical therapy.
This is shocking? Right? Here's the classic logic:
-ICH is bad
-ICH is worse if your platelets are iatrogenically broken
-Non-broken platelets are good
-Transfusion of non-broken platelets helps stop the bad bleed
