This 👇🏽👇🏿👇🏻 This is why #BLM exists & why “implicit bias” is not only real but it kills

Why, when you see it, say something, do something
The system changes not by sharing tweets & hashtags but actions & consequences & removal of toxicity from the system

Take #sharethemic initiative. It allowed #Blackexcellence to speak thru #influencer accounts to large followings for a day. I looked at before/after numbers, it did NOT drive any additional traffic to the actual accounts of those voices. Resurfacing this:

Issue brief THREAD: addressing #SDoH through #ValueBasedPayment #HealthPolicy

I'm SUPER excited about this piece. Work began ~8 months ago, I led it, a topic I care a LOT about (and which the Biden administration does, too). @DukeMargolis @MilbankFund… Image

Alright, finally had a day this week without 200 meetings 😅 so I have time to give this very important topic its proper twitter download. Tweets incoming.

#HealthPolicy #SDoH #PaymentReform #ValueBasedCare
2/ Preface: Important to acknowledge differences in language
-"SDoH:" structural factors shaping downstream social factors
-"Social risk factors:" individual adverse SDoH
-"Social needs:" patients' preferences for help w/social risk factors

Yes, the #vaccine rollout is slow, uneven, not matched to need. "Botched"? That is a bit too strong. Much blame to go around.

American #healthcare is built for silos, turf, acute care, reactive..not #populationhealth #publichealth #prevention. It shows. Why is anyone surprised?
And #MedTwitter, despite your hashtags & publications, when it comes to real life actions inside departments, hospitals, or even on Twitter, all you all do is turf, ego, cliques, us vs them, etc. I am unimpressed. Build *systems* Yes #impostersyndrome makes you crave validation.
I am a #pediatrician and if you need validation, I've got a whole box of stickers for you. I will even let you choose your own sticker. How is that? Because, yes, you are special. You matter. You are wonderful. Happy?

Then after that, could we get 💩 done?

Important piece in @nytimes summarizing the promise and challenges ahead for #telehealth by @ReedAbelson. Privileged to join the conversation.

As I stated here, despite the promise, there’s a lot we don’t know about #telehealth. THREAD…
We need to understand how it delivers value to patients by improving outcomes, quality, and affordability of care 2/
If the right hand is #telehealth, the left hand is #valuebased payment (including #PCPCapitation) - the method of how we pay matters a lot in motivating the behaviors we need to see in our delivery system. 3/

Thank you @ColleenKraft for being consistently vocal 💪🏼💪🏿💪🏽

Let’s work on #tweetiatrician #advocacy training by @AmerAcadPeds to have more robust & effective strategies vs increasing backlash & #bullying
as experienced by @NicoleB_MD @DrToddWo, worse for #minoritiesinmedicine
From @bfrist on #LinkedIn: concerning loss of scientific #integrity by political agendas

#scicomm teaching has been to “avoid politics” = sitting in sidelines

why engaged #tweetiatrician were a separate species apart from other #STEM professionals…
We need to be multidisciplinary like work with school nurse leader @RobinCogan

When I have taught official @AmerAcadPeds guidelines to trainees in hospital settings & staff member has walked by, they assumed “political” & “unprofessional” when per AAP

Umbereen S Nehal MD MPH: who is this person? Several people have been asking

A thread

Where am I from? Hooboy.
Let’s say, citizen of the world, human ping pong ball. Third culture kid. Genetic mutt

Born: 🇺🇸
🇵🇰 parents - when my parents born colonial rule, pre-Pakistan
To thoroughly annoy my Twitter bestie, @DrvanTilburg

Am an ENFP - a cross between Snoopy, Robin Willians, Ellen Degeneres, Dr. Seuss, Charles Dickens, Upton Sinclair

I border on ENTP - love new info and love to challenge in order to break through barriers or limitations
My great-grandfather was given title "Khan Bahadur" (brave). As a judge, a brown man under Colonial rule, he threw the book at a British attorney. "Learn the law" if in his courtroom. Hence my stiff moral backbone

(translation, some people aren't going to like this, and we don't really care)
3/ Our spending on Medicare and Medicaid has tripled since I was last at the Humphreys Building, and while I understand that sounds fantastic to all you hospital folks, I assure you, we're not happy about it.
