Cerebral atrophy (brain shrinkage) in an infant: Sad story with a happy ending

1. 10-month old infant was brought by his parents with generalized weakness & lethargy for 4 months. His motor milestones had also slowed down. He was evaluated by a local pediatrician.

2. Thyroid profile was normal. MRI brain showed severe, diffuse cerebral atrophy (generalized shrinkage of brain).
Anticipating a neurodegenerative disease, pediatrician referred the infant for a neurologist's opinion.
I took a detailed history and ordered for a blood test.
3. I reserved my opinion regarding prognosis, until the blood report came. I also did not want to send the infant for a series of investigations to rule out rare neurodegenerative diseases before seeing the blood test report.
Mother was a strict vegetarian, which gave the clue.

Shaking of hands and burning feet in a middle aged person: look for this treatable cause

1. 56-yr old Mr K presented with shaking of hands for several months, and burning & tingling of feet for 2 weeks. He had been started on trihexyphenidyl for tremors by a neurologist.
2. Clinical examination revealed action (postural) tremors. He had no slowness of moments, rigidity or balance issues.
Tremors were suggestive of essential tremors (ET). Burning feet was suggestive of sensory neuropathy. He had no numbness or weakness of feet.

3. Blood tests done earlier showed vitamin B12 deficiency. All meals consumed by Mr K (except 4 meals/month) were vegetarian.
NCV studies confirmed sensory neuropathy (damage to nerves of both feet).
Putting everything together, vitamin B12 deficiency is the most likely cause. ImageImage

Nervensystem (funikuläre Myelose) sowie einer Schädigung der peripheren Nerven (Polyneuropathie). Außerdem können zerebrale Störungen mit Entwicklung einer Demenz auftreten, auch mit Depressionen wird B12-Mangel in Verbindung gebracht. Diese Schädigungen sind teils irreversibel.

For reasons I still do not understand, my brain remembered something we are taught in primary school. Kwashiorkor is a disease caused by severe protein deficiency. Shamefully, children still die due to it in India. After getting reminded of kwashiorkor, I connected the dots.
I replied, "Kwashiorkor is still prevalent in India. By subsidizing the poultry industry, the govt is trying to increase supply of chicken. This will reduce prices of chicken thus making it more affordable to those who want to eat it. This can help reduce protein deficiency."

I posted multiple stories about the animal sources of vitamin B12 on WhatsApp status. I love to use Canva & Adobe Express to make stories about cobalamin. I also explained that processed foods such as Kellogs cornflakes, Chocos, Horlicks, Boost, Marmite, etc have vitamin B-12.
So this gent met me in person the same day. Looking at my status on his phone, he asked, "Is this verified news?" I was wide-eyed. Here was an engineer who had biology till class 12. He had completely forgotten that we had learnt about the animal origin food sources of vitB12.

Excess of vitamin B12 may be hazardous

1. Vitamin #B12 deficiency is known to cause anemia, peripheral neuropathy, dementia, psychosis and skin pigmentation. Vitamin B12 supplementation is needed in them, however, #vitaminB12 excess may have adverse effects on health and life.
2. In a recent study, >5500 people (mean age 53 years) were followed up for 8.2 years.
Those with high plasma vitamin B12 concentration had 25% higher risk of all-cause death.
The mechanism of increased death is unknown.

3. Older age, high blood pressure, poorer kidney function, as well as increased concentrations of liver enzymes, were positively associated with higher plasma concentrations of vitamin B12. So, these groups should be more cautious while taking vitamin B12 supplementation.
