🔸Risk scores may not accurately stratify younger patients

🔹Full risk assessment has to include with the bedside eval w/ imaging + lab markers as adjunctive data

🔸Be on the lookout for normotensive shock

🔸Contralateral arterial access during thrombectomy in case urgent MCS is needed

Key findings highlighted ⬇️
2/ #DOAC are now the standard treatment for many patients with #VTE or for those who require prevention of stroke and systemic embolism, particularly in the setting of non-valvular atrial fibrillation.
3/ A few RCTs compared #DOAC vs #VKA for treatment of patients with thrombotic APS and raised concern for ⬆️ thrombotic events with #DOAC compared to #VKA, particularly arterial thrombosis. Not sufficiently powered to assess individual thrombotic outcomes or to analyze subgroups.

Intermediate-dose versus low-dose low-molecular-weight heparin in pregnant and post-partum women with a history of venous thromboembolism (Highlow study): an open-label, multicentre, randomised, controlled trial thelancet.com/journals/lance…
We found that the higher dose did not reduce the risk of recurrent #VTE during the entire study period (Pregnancy plus postpartum period), which was unexpected and perhaps due to an over-enthusiastic sample size calculation…
A priori as a secondary endpoint, we noticed a reduction of superficial vein Thrombosis was included. This is important as this is a strong predictor of #VTE and prompts increasing the dose of #LMWH

Pulmonary #embolism (lung blood clot) was >10 times more common after #SARSCoV2 infection (COVID19; ~15x over background rate), vs after getting #Pfizer's or #AstraZeneca's vaccine (~1.2x> background)

Similar greater risks with COVID-19 for #DVT & #VTE

2) First of all – as the authors note – in those #vaccinated against COVID-19, "thrombosis, thrombocytopenia, and thrombosis with thrombocytopenia were very rare events."
3) Data from 1.9 Million (M) recipients of AstraZeneca, ~1.7 M of Pfizer, and ~300k COVID-19 cases (few old) – compared with ~2.3 M in the general population.

These cohorts were used to calculate the observed incidence vs. background (SIR) or observed vs. expected # of cases.
