The solution:
The dyssynchrony is equivalent to the septal flash of LBBB (in fact RV pacing is LBBB), but in this case limited to the apex. We se initial contraction in sept. apex, then contraction in sepal base and left apex, concomitant w stretch of sept. Apex. Finally recoil in sept apex.
It’s completely different from ischemic dyssynergy, where the dyskinetic area is delayed.
