We are on a mission to make health care better, safer, and more affordable by transforming the health care delivery system in North Carolina.
Step 1: Change the way we pay for health care.
One of our goals is that, by 2023, every @BlueCrossNC member will have a Blue Premier provider that is accountable for their total cost of care and quality. Accountable care = better care, better experience, more efficient care.
7 out of our 10 largest health systems are in Blue Premier. But another important leg of this effort is independent physician-led ACOs – powered by our partner @AledadeACO.
In short, we have one year of results – and @AledadeACO CRUSHED it! 💪
In year one of @AledadeACO and @BlueCrossNC’s collaboration in #NC physician-led Accountable Care Organizations (ACOs) ⬇️ total cost of care for commercial members by 4.7% and Medicare Advantage patients by 6.1%, and ⬆️quality across 47 practices.
These participating independent primary care practices earned $10.5 million ‼️ in incentive payments in their first year, providing a crucial financial lifeline to PCPs on the front lines of #COVID19.
Participating practices also crushed it on quality of care and health outcomes; practices improved their Medicare Advantage Star rating from ⭐️3 to 4.5 🌠 , through improved care for patients with chronic conditions like hypertension and diabetes, and more. Amazing!
@AledadeACO’s work enables clinicians, practice leaders, and @BlueCrossNC to work as a team and leverage technology to improve the quality of care for patients, better manage chronic diseases, and ultimately reduce health care costs. @Farzad_MD
