Sodium laureth sulfate clogs your pores if it comes in contact with your skin. If this ingredient is present in your tooth paste and you foam out while brushing, you are likely to breakout/develop whiteheads and pimples around your lips.
2. Your phone touches your face too much.
The germs on your phone is greater than the one on your toilet seat (if I ain't exaggerating). Which literally means you holding and using something that's dirtier that shit.
Now imagine letting this phone touch your cheeks.
If you spend long hours on call, sweats come in contact with the germs on your phone, then contaminate and clog your pores and you break out terribly.
The best thing to do is to sanitize your phones.
Clean them everyday with a sanitizer containing alcohol.
3. You touch your face too often.
This is the most common and almost everyone does it. Your hands is like a middle man between germs and your face.

Always wear sunscreen. Always.
Sunscreen can reduce hyperpigmentation and dark patches on your face.
