(1) There should be a lot of people screaming "WTF" after reading the @WMATA stories by @tomroussey7news @AdamTuss & @justingeorge on the March 26 debacle at Rhode Island Ave. But a major safety issue is missing from the news reports & apparently in Wiedefeld's memo. (more)
(2) There's no acknowledgment of another major break down in communications between ROCC & @OUC_DC. The inability to relay timely emergency information is a big part of what killed Carol Glover in 2015. It's painfully obvious these failures continue to this day. (more)
(3) Because of poor communication between ROCC & OUC, @dcfireems was first dispatched for an EMS call at R.I. Avenue. Then for people trapped on an elevator. And then for a decoupled train. All of this wrong info evolved over 16 minutes. Why? How does this happen? (more)
(4) I've been asking @wmata, @OUC_DC & @SafeDC regularly & unsuccessfully since March 26 for a simple timeline of the interactions between OUC & ROCC. None of them are showing transparency & accountability on this important issue. (more)
(5) As they should've finally learned from Carol Glover's death on Jan. 12, 2015, getting fire & EMS to the right location of a rail emergency & armed with the correct information MUST be a priority. If not, people will die. How come Metro & OUC remain such slow learners? (more)
(6) Also, I was astounded by this line in @justingeorge's report. It's inconceivable @wmata's safety people weren't told or didn't know on March 26 that 2 passengers left the train on their own & walked the electrified tracks. I knew that info on March 26 as did others. (more)
(7) Both @MetroReasons & I reported 2 passengers had apparently left the train & were on the tracks within a couple hours of the incident, a dangerous situation. For the safety of riders, Metro should've also revealed this on March 26, warning passengers of the danger. (more)
(8) Instead of immediately getting a crucial warning to its passengers, Metro just let it leak out over the weekend. After all of @MetrorailSafety's great work & @WMATA's commitment to change, it's clear "Safety Trumps Service" is an empty slogan.
