Paul Coelho is openly committing CMS fraud by falsely dxing patients with OUD so Suboxone will be covered. He talked about it on camera. In fact, when you hear the term “complex opioid dependence”, this is an effort to get around off-label use for pain w/a new dx. @OIGatHHS

The @ORmedicine doesn’t care that this MD calls ppl with substance use disorder “drunks” or intractable pain patients “lotus-eaters”, citing Dr Coelho is protected due to free-speech. Imagine how promoting stigma on social media harms ppl. They’re protecting an abusive MD.

At 12 min 40 secs, talking about tapering ALL patients above cdc guideline limits, and 13min 15 secs, when the forced taper flounders, then he talk to them about an OUD dx and bupe. Hes’s saying to use the symptoms of WD to initiate an OUD dx, b/c of difficulty w forced taper.

At 8:15, he acknowledges that the DSM5 criteria is not being met.

23min 50sec: “When you come to this clinic and you’re a high-dose patient, you’re gonna taper and you’re gonna go to buprenorphine.” Also, he stated once RXed bupe, you’re forever banned from pain meds. As seen in this screenshot, there is no reversing any taper, no matter what.

29min 50secs: “When the patient fails the taper, if they’re below 1000 mmes, we’re going to rotate to bupe and it will be bupe forever after, and I will diagnose OUD.”

One of the most offensive things about Coelho is the way he and Jane B. refer to pain patients that have taken over 90 mme of pain meds as “The Lost Generation”. Talk about stigma and discrimination!
