The Story of A Physician.
This physician 45 / F was fine in 1st wave treating patients, second wave ( Delta) she was infected in March 2021 ( was fully vaccinated - AZ) was had just little cold for 4- 5 days then ok. Ctd)

Ctd 2) Feeling more confident she & her family took a short trip over new year , all infected by 7th of Jan 2022 she started showing 1st signs of infection ( RAT+ve) and on 15th Jan 2022 she went into a full blown cytokine storm . Hospitalized &given Remdesvir ( episode 2)ctd)

Ctd 3) thankfully her O2 levels stabilize & discharged in 5 days .Her lung shows a residual ground glass appearance around 3 weeks later , the ID specialist considers that she is now developing too much residual issues & starts discussing antifibrotic therapy / steroid. Ctd)

Ctd4) she is now 1 month of infection( episode 2) .She visits her hospital , is coughing intermittently ( everyone feels it is post lung fibrosis ) has tea with some colleagues all remain unmasked for the tea , others visit her home that day. 2 days later develops fever.ctd)

Ctd5) her RTPCR is positive( again) , 2 of her colleagues have fever ( again having had it last month ) and now she is being considered for Molnupiravir/ Remdesivir(Had one course last month) pending D Dimer levels. Have asked her CD4/CD 8 evaluation ctd)

Ctd6) Though she mounted adequate response to Delta BUT the episode 2( Omicron/ Delta) and the new RTPCR positive after just a month ( with 60% lung already involved ) means we need to be extra careful , all in all may have a difficult situation on hand. Ctd)

Lessons -
1) The Pandemic is not over yet .
2) Continue wearing N95 even with colleagues.
3) Delta infection/ Omicron infection doesn't mean that you have developed immunity forever.

Update on the patient - When the thread was posted she was so dyspneic that she could barely talk on phone .Her Crp and D dimer was normal 5 days ago , now her D dimer was normal BUT within 2 days her Crp jumped to 32.She was put on Molnupiravir, her condition became better ctd)

At day 5 ie today - Her CRP has come down to 20( from 32) she is able to move in the house without dyspneic. She is still unable to climb a flight of stairs .Her physician has put her on Anti fibrotic medication as well.Definitely a case of reinfection. May need Fluvoxamine.

Just a clarification- I am just reporting this case and not the treating physician in any case . So my views and treatment protocols are immaterial . This case was discussed to bring home the risks of reinfections with Omicron, have had 2 in last 1 week .
