Epidemic of carbapenem-resistant Klebsiella pneumoniae in Europe is driven by nosocomial spread | Nature Microbiology nature.com/articles/s4156…
As compared to other resistance mechanisms for Klebsiella pneumoniae, strains with carbapenamases were more likely to have their “genetic nearest neighbor” within the same hospital, suggesting they are more likely spread within single hospitals. #TwitterIDJC
Over half of the hospitals that submitted isolates likely had within-hospital transmission #TwitterIDJC
One point brought up by our group is that the study did not look at community strains, so it’s possible these strains are in the nearby community but not necessarily being spread just in the hospital. #TwitterIDJC
Starting a new thread for our discussion of the clinical article
Another criticism about this article is that it plays down the “One Health” model of taking into acccount environmental spread of carbapenamase-resistant organisms

1/ Your team just saw a patient with syphilis, and you're ready to teach, but:
Resident #1: on week 2 of their rotation
👉 Has already seen 2 patients w/ syphilis
Resident #2: started today
👉 Hasn't seen a single patient with syphilis
What should you do now?
2/ Unfortunately, learners on our team may miss teaching that occurs during the rotation for multiple reasons.
3/ As @VarunPhadke2 previously pointed out, all learners on the team are usually not present all day, every day for the entire time we are on clinical services.
